Management plan

Management plan

Has anyone had a GP complete a Chronic condition management plan?

if yes,

- does patient need to sign it?

- does GP need to include reason/s for referral to specific allied health provider?
- does patient need to have a copy prior to contacting the allied health provider in their area?

- does the referral only go to the nominated local allied health provider ie patient not seeing what has been recorded within the management plan?

Re: Management plan

On GPCCMP to podiatrist GP has recorded the name of a specific medication. Google search shows me it is the combination pack of antibiotics I’ve recently taken for the week duration. It is script only. GP has recorded the dosages as “as directed”.

Only reason this combination pack of antibiotics would be issued is if, at some stage, blood tests results shows positive for a specific virus, again. I fail to understand how that med can be classified as ‘current’

Re: Management plan

Thanks for the updates @Flutterbug9 - sorry to hear that the blinds is an ongoing stressor for you. Can be such a pain/burden to have to keep chasing up stuff like that.

I'm glad you and your psych have built a relationship where you feel you can be assertive with them about your needs. I know sometimes there can be a bit of a power imbalance, and we can feel like the therapist is the expert and thus know what is best for us. But we are the only experts in our own lives - I hope the conversation goes well.

Frustrating about the medications being in such a jumble, and having the antibiotics listed erroneously by the sounds of it. It sounds like you're having to do a lot of untangling of other people's errors or missteps. All of which becomes an extra mental load, hey.

I hope you find some respite soon hun 🤞💜

the power imbalance was how I felt with the previous psychologist. Was subconscious, thinking back I’ve realised I viewed the psych as being in a position of power. At times words and tone added to that feeling.

the new psych during our first appointment asked if I was to provide any info for reason/s for transfer request in hopes to avoid anything that had happened with previous psych.

One of my struggles with previous psych was things had to be exact eg. Boxes a,b,c,d,e with contents 1,2,3,4,5. When I put the contents into the boxes in order of 1,2,4,5,3 the immediate response was that I was wrong and I was then told what exactly why I must put content 1 into box a, content 5 into box e etc. there was never any discussion about why I put them in the order I did. (One of the times the psych triggered me).

My experience with new psych is the total opposite. She asks questions …. “How did xxxx make you feel”, “why do you like that’s xxxxx” and similar questions. An approach, which I said in the last appointment, I’m comfortable with. Gives me a sense of being invited to explore my beliefs, my feelings etc.

our last appointment I was feeling uptight, on edge, full of nervous tension and was hoping to have talked through, at least some of, the causes. I said at start of the appointment how I was feeling, walking to my car after the appointment I realised what I wanted to talk through didn’t happen.

At the end of previous 2 appointments psych said we would continue discussion on topic xyz next time, didn’t happen either time. Current psych has been made aware by me that happened with previous psych and I don’t want it to happen again.

current psych client group commences with children and includes all age groups thereafter. Her bio mentions experience with trauma, ND, DV, SA, school refuses.

@Flutterbug9 Hello lovely how are you?

Im sorry you’re having to sort out the mess in regards to medications not being listed correctly. It causes so much stress and it should just be done properly by doctor so that all medications are correctly on file.

This would allow other treating specialist etc to know what you are actually taking. I can totally understand how frustrating and upsetting it is.

Re: Management plan

I now have a spreadsheet document on my laptop which I update when required @Cuddlebear

;nothing eloborate. 2 columns, one with name and other with dose info (eg vitamin c, x number once each day).

I found it was easiest option when filling out medical related forms. When I go into hospital, I have the spreadsheet in the travel bag with my meds. Admission Dr in December last year said it made their job much easier.

All my specialist have asked me to advise them what meds I currently take, both script plus OTC, even though my GP has provided them my health summary.

doing the spreadsheet provides me piece of mind plus stops thoughts racing in my head.

Re: Management plan

@Flutterbug9 oof, yeah I would struggle a lot with a psych like that! Especially over something that seems so minor... Maybe there's people out there who would benefit from an approach like that but I can totally understand why you were triggered by it.

Do you feel like you still got something good from the appt, even though you didn't get to talk about what you wanted to?

Re: Management plan

I remember sharing more of things that previous psych did that I don’t want to happen again @Jynx plus how those things left me feeling

mentioned to my new psych reasons I like her approach and how it has made me feel, both being positive experiences.

no idea what we talked about, which is unusual for me.

I am looking forward to the next appointment and feel comfortable within myself to bring up the topic, explain the couple of things that have happened that I’m not feeling happy about. Overall context, they are minor. Don’t want things to happen like with the previous psych and this one opened the door asking me to share with her things that happened last year on a negative aspect.

End of one appointment I mentioned about having unusual hobbies. We had run out of time, new psych said we would continue that chat next appointment and the AI program that is used during my appointments will help prompt her memory. It didn’t happen.

minor but has potential to grow, something I want to try to stop before anymore time goes buy.

@Flutterbug9 this is a good approach with most 'minor' things in my opinion. It may seem minor in the moment and not worth addressing, but small irritations can build to resentment when left unchecked. It's really good to hear that this psych is so open to feedback - a very desirable trait for a psych I reckon. It is your journey, and we should all have the ability to give direction or highlight what's not working, and not all MH professionals know how to open that door to let us feel like we can approach these things.

How long till the next appt?

Re: Management plan

1 week to next appointment @Jynx

It’s nice interacting with MH specialist whose tone and facial expressions show they are “with me”.

my SI has already come up in discussions. I saw a face with which there was a smile to a very sad, serious face. Silence hung in the air for short while. Tone in the words that were said were that of someone who had been stunned.

Re: Management plan

How does that sit with you @Flutterbug9, that she was 'stunned'? I know some folks find it validating, others find it tiring...
