Management plan

Management plan

Has anyone had a GP complete a Chronic condition management plan?

if yes,

- does patient need to sign it?

- does GP need to include reason/s for referral to specific allied health provider?
- does patient need to have a copy prior to contacting the allied health provider in their area?

- does the referral only go to the nominated local allied health provider ie patient not seeing what has been recorded within the management plan?

Re: Management plan

I think I had one for physio for my shoulder @Flutterbug9 but it got better and I didn’t end up using it. But pretty sure I had to sign it. And there would be a reason for the referral otherwise how do they know what the issue is? And yes I have a copy of the plan

Re: Management plan

I have a Chronic Condition Management Plan @Flutterbug9

I didn't sign it, but I agreed to its creation and it was written with me in the room. It's for the management of my mental health conditions and anything to do with my past diagnosis of melanoma (skin checks etc). I don't know a lot about it, except that it allows the GP to bulk bill for certain appointments related to the chronic conditions on my management plan.

I can't really answer your questions, because mine doesn't involve referral to allied health professionals.

Re: Management plan

Due to severe spinal injury in early high school/secondary I have ongoing issues with my feet @NightFury

GP asked if I use the 5 free visits through Medicare. I’ve been with this GP for more than 2 yrs, first time mentioned. To me, answer would obviously be “no”.

GP asked which allied health provider, I said podiatry

apart from telling me referral would be emailed to me with details of business to phone once have the referral there was no other discussion. That was Thursday last week. As of 1pm today no referral.

cost of fuel, parking plus consult fee 4 times a year, it gets expensive

Does that help answer your questions? I think that's the legal stuff

@Flutterbug9

I can help!

A chronic disease management plan allows you to see an allied health professional for issues that require maintenance rather than acute treatment.

Every year, resetting on the 1st January every year, you are allowed 5 discounted or bulk billed treatment sessions. This breaks down into 1 session every 10-11 weeks.

  • does patient need to sign it? not from my previous experience, but they have done a recent overhaul.
  • does GP need to include reason/s for referral to specific allied health provider? yes, though not so much a specific provider, but a specific type of provider, like in my case, physiotherapy
  • does patient need to have a copy prior to contacting the allied health provider in their area? This depends on whether you've chosen them or your doctor has. Some GP's will send them a copy for them to make first contact or give you a copy to approach them with. My doctor does both, sends my physio a copy and gives me a copy for my records.
  • does the referral only go to the nominated local allied health provider ie patient not seeing what has been recorded within the management plan? If this happens you are well within your rights to request a copy from either your GP or the provider that contacts you. The plan should be completely collaborative and have full transparency with you. Any failure to do so is not ok. You are also able to request a copy to find a more appropriate provider within the same field, like if you found one that bulk bills rather than partly paying for the person your GP sent you to. I don't know about switching providers part way through a referral though.

Re: Management plan

Thanks @avant-garde @for all the info.

I was searching for the info in the screenshots, I had no luck though

this is 3rd referral from GP, first being early December last year, where I’m having to chase things up

Re: Management plan

When I got my plan, I signed it, took it to the podiatrist and I made the appointment myself. The clinic gave me the number and I booked it then gave him the plan @Flutterbug9 But that was quite a few years ago.

Yes, and only the person I gave the hard copy document to, had the record.... it didn't go to other allied health providers.

Re: Management plan

Venting only - not tagging anyone

im annoyed and frustrated. This afternoon I received email from my GP with letter attached which was emailed, at same time as to me, to a local podiatry business.

referral letter states “comprehensive care plan attached”, my current medical conditions, “management goals and ongoing treatment strategies”.

1) only item attached to the email is referral letter. Comprehensive care plan no where to be found

2). I fail to understand how a GP can do the care plan etc when only thing I said was “referral to podiatrist would be helpful”

3) I already attend a podiatry firm who I see on regular basis for care on my feet. There was no discussion about which podiatrist firm I would like the referral or care plan addressed to. GP has made an assumption and has referred me to, for me, a new podiatry firm that is less than 5 mins from where I live

tonight I’ve sent reply to GP via email asking if the comprehensive care plan mentioned in the referral letter was to have been attached to the email as well. I know in the letter it says “attached”, deciding to try to keep this as pleasant as possible with GP.

if there was more GPs in my area and GPs that are accepting new patients I’d be seriously thinking about transferring.

Re: Management plan

Apologies in advance, I’m feeling annoyed and frustrated

As in my vent, I’ve finally got referral letter from GP @tyme and only a referral letter which states comprehensive care plan attached.

ive been attending one podiatry firm for more than 20yrs by now and see one of their podiatrists each few mths (non diabetic issues).

There was no discussion about which firm referral to go to. Referral is addressed to a firm I’ve never been to. Is heaps closer to where I live, to me that’s not the point

though

ive got appointment with psychologist tomorrow morning. Might vent with her if I haven’t calmed down by then. I could possibly go to MH drop in centre and vent. Approx 45min drive at 11pm at night, no thanks
