Management plan

Management plan

Has anyone had a GP complete a Chronic condition management plan?

if yes,

- does patient need to sign it?

- does GP need to include reason/s for referral to specific allied health provider?
- does patient need to have a copy prior to contacting the allied health provider in their area?

- does the referral only go to the nominated local allied health provider ie patient not seeing what has been recorded within the management plan?

Arghhhh @Flutterbug9 ,

How annoying!!!! I hear how stressful and frustrating this all is for you! And it's not that access is the easiest for you.

Oh @Flutterbug9 , I wish I could swish out my magic wand and make it all better for you!

Hugs

Re: Management plan

This last 10 days has been one thing after another. Plans being changed short notice, numerous phone calls and emails trying to chase things and get answers.
Electrician at my place on Monday told me it would cost thousands to be able to get 2nd external power point, also said he doubted the work could be done.
today email and phone conversations with senior staff member at electrician firm providing me quote for few hundred and explaining how the work is to be done.

got off the phone with the senior staff member and I was shaking. not just feeling it inside, my hands was physically shaking. Proverbial straw that broke the camels back.
Psychologist appointment couple hrs later and that anxiety feeling in my chest still hadn’t gone away.

various contacts with firm that I’m paying to install my alfresco blinds with conflicting info in the calls. Final call from person who did the measure and quote gave me good news and was my final undoing. Tears silently rolling down my face. Nothing left in me to actually cry. Been in tears and feeling anxious and edgy for days now.

still waiting on GP plus one firm to contact and provide me with info.

I can’t take much more. Driving home after being out for the evening, so many black thoughts in my mind. I’m safe, wish I wasn’t though. I’m tired of life and don’t want to do this anymore

Re: Management plan

Phone call this morning from local podiatry firm, they had received referral letter plus the care plan. Have had initial appointment with one of their podiatrists who provided me with lots information about my feet, info no podiatrist has done before (that I remember)

receptionis happily provided me a copy of the care plan whilst I was there

Re: Management plan

Hey @Flutterbug9 ,

Your recent days sound hectic in terms of the electrician etc.

At the same time, I read about the podiatrist. I see the change as having been beneficial to you. Am I right in assuming this?

Getting a specialist that really cares and is willing to keep you informed, goes such a long way...

Re: Management plan

Podiatrist was very good today @tyme

unexpected phone call and appointment set my emotions into a spin and completely upset my plans for today.

was a positive outcome including, most likely, no more surgeries

lots of highs and lows, constant changes, panic attacks, emotional upsets and not much sleep these last couple weeks caught up with me this afternoon. Never good when wake from nap not realising how tired one is. Struggling to keep my eyes open. I’m off to bed

Re: Management plan

Glad to hear the podiatrist was very good. Thank God to took that leap of faith @Flutterbug9

All those highs and lows sound exhausting.

Take care. Will catch up next time.

Re: Management plan

Sounds like things have been really tough for you @Flutterbug9 lots of mental load to sort things out, deal with last minute changes, it's no wonder you're exhausted.

Hope this week has been a bit lighter for you so far?

Here for you 💜

Re: Management plan

Finally getting some definite answers and steps that things need to be done in @Jynx which is being a relief.

im not fully happy with person from the blind company who did the measure. I requested quote be update to reflect plscenent of motor of both alfresco blinds has had to be changed as is no longer as stated on the quote, person has politely refused.

couple of things I need to talking to my new psychologist about at next appointment in hope of bringing conversations back along lines I stated at first appointment I want. Nothing major, something I want to keep on top of though.

next appointment with GP, which will be in May, I’m hoping to finally get my current medications updated and corrected. Every referral shows I’m taking 2 different anti depressants whereas I’m only taking 1. Has been well over 12mths since I’ve taken the other one. Somewhere within the last couple of months I printed off spreadsheet I’ve drawn up which shows name of each med (including all OTC meds), their dosage, when and how many I take, nothing has been done following me providing that list. In May I’ll be taking in all my meds plus another copy of the spreadsheet.

Re: Management plan

@Flutterbug9 did you tell your GP that you already had a podiatrist? If you didn't, it makes sense they would make a recommendation for you.
