Managing thoughts of suicide & self-harm

TW: The 'real' me inside - I come with an SI warning

TW: The 'real' me inside - I come with an SI warning

I figure it's better to introduce my challenges here than in a welcome. Hopefully only people willing to hear about SI will venture here.

TW: Suicidal Ideation

Content/trigger warning

I've had thoughts of suicide since I was 12. I don't remember much of life before this time. I'm aware there are times that I haven't though as much about suicide as others, but it's always been a "yes please, that would be my preference" even during the better times.

Why am I still alive then? For two reasons:

- the emotional pain it would cause to very close family.

- the physical pain of accessible methods, and the risks of non-fatal attempts causing lifelong physical impairments.

How do I keep myself going all these years?

By focusing on how I can benefit other people's lives instead. If I'm going to put up with life, it may as well be beneficial to someone.

And to help me keep life less monotonous I look for 'interesting' things - quirky people, exploring foreign countries/cultures, trying new foods (even if i dont think I'll like them). I've never been a status-quo person.

I explain to MH supports that when life is not distressing I can carry on without seriously considering suicide as a possible option. I can say "hello there thought, I hear you but I have chosen not to act on it, things are OK". But when something sets things off kilter, its a pretty quick track back to "why am I still doing this? Do I really still have the same commitment to living? And why, for whom? Why can't I be permitted to die if that's what I want for myself?"

With decades of this under my belt, I have the ability to sit with feelings of extreme distress. And the courage to text or call someone if I need to - and I'm getting a little better at doing this *before* it gets too extreme, but I still hesitate because I don't want to burden others and the frequency of it happening during those unbalanced times is too often. I've persuaded myself a few times recently to instead call a crisis line, but waiting 30-45min in a queue can sometimes exacerbate the thoughts, and sometimes the calls might help dissipate the feelings (if i called early enough) but other times I feel worse after because of course they're going to try to give some positive outlook for the future, but it grates on me because deep down I dont want a future at all. Its actually refreshing when someone acknowledges that not liking/wanting life is OK - and instead just tries to look at how to rebalance the scales to help me cope with continuing to live despite not wanting to.

I'm not sure how un/common my way of thinking is. So much about suicide prevention is focused on short-term distress tolerance or "fixable" problems (eg situations).

What do I need from the community here...?

To be heard? To know if anyone else out there has a long enduring preference to die even when they aren't feeling active distress? And if so, has anything helped them longterm to shift that thinking?

If anyone knows of any practitioners who *specialise* in chronic SI since a young age, I'd really love to know who (if there is a way someone is allowed to??). I've tried Google searching and using online psych directories and reading bio's. I've seen many psychologists and psychiatrists in the past who haven't been able to help me shift this (even if theyve helped in other ways).

I do wonder if my neural networks for positive emotional may not have fully developed or may have atrophied due to the young age and length of time. I genuinely do not know what being happy feels like, I do not have any memory that I recall as being "happy" or joy, and I suspect this makes recovery much harder.

Im ok at the moment. Im safe. But it is exhausting.

Apologies this post is quite a bit too long.

Financial areas is something I try to not talk about on the forums. Few different things pain management Dr said on Friday regarding pattern of different treatments and knowing approx out of pocket amounts for each one, I went into panic attacks.

I’ve got a head for dealing with finances, was one of the reasons I started accounting course at TAFE. Usually can sit and number crunch easily, actually helps distract me. End of this week I’ve got person coming to do free measure and quote for part of something I would like to get done. I don’t cope where outcomes are unknown.

Finances of my own medical treatments and cattery costs, unknown costs for work on outdoor area, unknown status of health etc for relative with cancer, her cancer opening scar and leaving raw, sore spot due to memories of my husband fighting cancer 6yrs ago. Hence I tend not to answer when I’m asked how I’m going. It’s all too much atm @Cuddlebear.

know I can be open with and talk to you, @AlwaysMyself @and couple other people on the forums. At times try not to dump everything, everyone dealing with their own things.

Do you enjoy word games online like Word With Friends? @Shaz51 tells me you can play with strangers, so you don't even need to know people first. --- yes that is right @Cuddlebear , @AlwaysMyself

you can play in a team like I do

or you can play without being in a team

I did well in my accounting subjects too @Flutterbug9 , it is a very useful skill i agree. I also hate not being able to predict/plan and be financially secure. Cost of medical (incl MH) care are so expensive out of pocket ☹️. I know thinking about how i would afford the amount of supports i need has sent me into a spin more than once before. I can completely understand how it could induce panic attacks.

It sounds like you have a lot of large stressor on your shoulders currently @Flutterbug9 . That's not easy. I'm glad you know you can share with us here safely; its not a "dump" and its not "too heavy" for us. Helping support those i care about (which includes you) is a positive thing for my MH, so in that sense you are helping me too.

Sometimes doctors will reduce fees if they know a person has significant hardship. Knowing your other medical costs and chronic conditions, i wonder if your doctor would consider it? And/or vouch for your need to other doctors if its their fees. I guess it can't hurt to mention that you are struggling with the potential costs? Opens the door for them to discuss options for reduced fees if they feel willing to / want to help. Or even if they know of other doctors who can do the treatment who would be willing to do a cheaper price?

I wish there was a way people could access interest-free loans from the government for medical care out-of-pocket expenses. Just adjust per year by CPI only. No pressure to pay back until can be afforded (similar to HECS debt in that way). Given than medical treatment can improve ability to work, and that means more taxes and less social welfare payments, i reckon it could even be financially not too burdonsome... 🤔

I did know know you can play as a team @Shaz51 ! How does that work?

Is there any other online activities you would recommend to people as ways to meet, connect and make friends with people? 🤔

Re: TW: The 'real' me inside - I come with an SI warning

@AlwaysMyself specialists and doctors who work in private practice can choose to charge whatever they want to. Medicare refunds are based on schedule fees. Every dollar over schedule fees are 100% out of pocket.

eg full cost of RFA and anaesthesia came to $x,xxx (thousands). After Medicare rebate I was still out of pocket by approx half which was more than $1,000)

I can’t do anything about costs charged by cattery.

starting costs for every cattery in my area is $37 per cat, per day. Research for sitter to stay overnight at my place shows more than $80 per day.

My GP already bulk bills me. I have private health insurance with all fees charged by private hospital being covered by health insurance firm.

Limit is 30 players on each team @AlwaysMyself all the points made of all members on the team played are added all together to make a total score

Re: TW: The 'real' me inside - I come with an SI warning

@Flutterbug9 sometimes specialists will be willing to bulk bill (or lower fees) on a case-by-case basis. I used to work for a psychiatrist who had different rates based on financial hardship (bulkbill, concession, normal rate, insurer rate). I've known someone who was quoted $10k's for cancer treatment, but ended up finding a specialist who agreed to bulkbill it for them.

I do understand the stress and distress of medical and supports costs ☹️. My weekly medical & supports (not medication or gp or adhoc specialists) cost more than a centrelink dsp pays - so even if i end up on a dsp later down the line, i will still be financially dependant upon my mum. 😣 however i am so lucky that she wants to help support me financially. If i didnt have than (and before i was able to feel acceptance of her decision to) i was very distressed and not able to see a way to afford life once my savings are all gone after my sick leave runs out. My last big spiral of SI (rather than just my normal SI) was because of this a month ago.

I hope the quotes come back to you soon, so that you can assess the costs vs income/saving and what things you are then able/prepared-to proceed with.

For many years (decade+) i have not sought medical help for things because of the ongoing costs (not a once-off fix). Its a hard choice to spend the "emergency fund" savings on treatment costs, esp without a guarantee things will work/help.

🤗 sending hugs for you during the stress of uncertainty and difficult decisions.

@Cuddlebear @Shaz51 @Flutterbug9 @Chasingsunsets

Person came over this morning to chat to me about the at-home support service their service can offer for up to 8 weeks (1h/wk) (government funded). I felt they understood, and they mentioned their experience of depression & SI was quite similar and they noted a few things they've seen through their personal experience and the MH workplace experiences that I thought were quite insightful. Shame she is the assessor/manager and not the support worker herself 😅 lol. But she thinks there is someone who (if available) would be a good fit with being flexible in adding to my supports for the short-term of the program. I will wait to hear.

@AuntGlow Not sure when you will see this tag (I know things can be busy), but I saw you are online via other posts. Whilst I am safe, I can feel my mood heading downhill. My mother is home, but with a friend visiting downstairs. I sat and chatted with them for a short bit, but grew tired of it so I have moved back into my area of the house instead. I don't want to interupt her. But even if I did, not sure what I would say / what she could do... "I feel low, and I think my brain is heading towards more intense SI again. Can you keep me company?" 😐

Thinking of doing painting again, but I know it won't take my mental focus away from thoughts, which could mean it still spirals downwards if that is where it is going.

I could put on a TV show, but I'm not that interested in them. They're just a way to pass time mostly.

I don't feel overly emotional or distressed (yet). But I can feel... (what can I feel?) .. the emotional fatigue and apathy. ...and my chronic reflux pain in my chest (it's been the background to my days for the past month lol, not a difference that triggers my mood).
