Managing thoughts of suicide & self-harm

TW: The 'real' me inside - I come with an SI warning

TW: The 'real' me inside - I come with an SI warning

I figure it's better to introduce my challenges here than in a welcome. Hopefully only people willing to hear about SI will venture here.

TW: Suicidal Ideation

Content/trigger warning

I've had thoughts of suicide since I was 12. I don't remember much of life before this time. I'm aware there are times that I haven't though as much about suicide as others, but it's always been a "yes please, that would be my preference" even during the better times.

Why am I still alive then? For two reasons:

- the emotional pain it would cause to very close family.

- the physical pain of accessible methods, and the risks of non-fatal attempts causing lifelong physical impairments.

How do I keep myself going all these years?

By focusing on how I can benefit other people's lives instead. If I'm going to put up with life, it may as well be beneficial to someone.

And to help me keep life less monotonous I look for 'interesting' things - quirky people, exploring foreign countries/cultures, trying new foods (even if i dont think I'll like them). I've never been a status-quo person.

I explain to MH supports that when life is not distressing I can carry on without seriously considering suicide as a possible option. I can say "hello there thought, I hear you but I have chosen not to act on it, things are OK". But when something sets things off kilter, its a pretty quick track back to "why am I still doing this? Do I really still have the same commitment to living? And why, for whom? Why can't I be permitted to die if that's what I want for myself?"

With decades of this under my belt, I have the ability to sit with feelings of extreme distress. And the courage to text or call someone if I need to - and I'm getting a little better at doing this *before* it gets too extreme, but I still hesitate because I don't want to burden others and the frequency of it happening during those unbalanced times is too often. I've persuaded myself a few times recently to instead call a crisis line, but waiting 30-45min in a queue can sometimes exacerbate the thoughts, and sometimes the calls might help dissipate the feelings (if i called early enough) but other times I feel worse after because of course they're going to try to give some positive outlook for the future, but it grates on me because deep down I dont want a future at all. Its actually refreshing when someone acknowledges that not liking/wanting life is OK - and instead just tries to look at how to rebalance the scales to help me cope with continuing to live despite not wanting to.

I'm not sure how un/common my way of thinking is. So much about suicide prevention is focused on short-term distress tolerance or "fixable" problems (eg situations).

What do I need from the community here...?

To be heard? To know if anyone else out there has a long enduring preference to die even when they aren't feeling active distress? And if so, has anything helped them longterm to shift that thinking?

If anyone knows of any practitioners who *specialise* in chronic SI since a young age, I'd really love to know who (if there is a way someone is allowed to??). I've tried Google searching and using online psych directories and reading bio's. I've seen many psychologists and psychiatrists in the past who haven't been able to help me shift this (even if theyve helped in other ways).

I do wonder if my neural networks for positive emotional may not have fully developed or may have atrophied due to the young age and length of time. I genuinely do not know what being happy feels like, I do not have any memory that I recall as being "happy" or joy, and I suspect this makes recovery much harder.

Im ok at the moment. Im safe. But it is exhausting.

Apologies this post is quite a bit too long.

@AlwaysMyself that makes sense, i think subconsciously, it's pretty natural to have some sort of expectation around things.

i've got my fingers crossed that you have a lovely time on this trip too. would love to see scenery pics (only if you feel comfy sharing) once you're back 💙

oo and you're going melb after too! yep the flights are sooo expensive, i think trip.com has some cheap ones at times but not sure i trust the app since i haven't used it yet. sounds like a jam-packed weekend!!

Hello! @AlwaysMyself,

How are you feeling today? 🫶

I realised I still needed to respond to your last message re: anhedonia. Once again, I appreciate how much insight you provide us, it's so helpful.

I am curious to know what your professional supports have said about your experiences? 🫶

Re: @AuntGlow What do you mean by 'feel connected in my body'?

Do you mean in relation to how/where I feel things in relation to emotional reactions? (If not, I don't know what you are referring to, sorry! But I know you will explain if so 😊).

Yes, that's what I mean! But it sounds like you are connected to feeling and sensation, it's just not always landing in the ways you're needing... I am so sorry you're experiencing a disconnect from joy, it must be really disheartening feeling this way. I wish I had more answers... maybe we can do some research together? 💛

@rav3n yes, happy to share photos on my return. 😊

@Cuddlebear thanks 😊. From the other chat thread, our sisters sound a little similar too. Another commonality between us lol.

@AuntGlow None of my psychologists, psychiatrists or counsellors have given a theory on the underlying cause or what might help (given the things that I already try/do). I'm very open to research/theories/ideas.

@AlwaysMyself How are you? What’s on for you today? You must be busy getting ready for your trip.

yes our sisters do sound similar.

Have you been having more counselling how is it going?

me I see psychologist this morning. That’s hopefully going to go well. I’m feeling pretty crappy and I’m hoping she can be of some help.

Re: TW: The 'real' me inside - I come with an SI warning

Im sorry youre not feeling good today @Cuddlebear , and hope chatting to your counsellor helps.

Is it something you'd like to chat about here? (You always can if you want to, but its also perfectly ok if you dont wish to).

Yeah, been seeing the counsellor weekly still. Its good to have someone i can talk to honestly and openly about the thoughts I have and my thoughts about life/work changes next year. So far no clear path/ideas for how to improve things in the short term (or perhaps long term either, other than trying out not working).

Dogs just got back from the dog walker, so theyre happy and will sleep whilst I head out soon to my counsellor and then massage.

I do need to start gathering items to pack to take away. Went to the chemist yesterday so I have my medications ready, incl some anti-gastro which we always hope to not need (better to have it and not use it, than to need it and not have it!!).

Going to pack as light as I can, because for some flights I only have 10kg of checked luggage (need to see which of my cases is the lightest one to take!!)

Re: TW: The 'real' me inside - I come with an SI warning

@AlwaysMyself Good morning lovely how are you? How was your day yesterday?

Im glad you’ve been seeing the counsellor. It does take some time to be able to work through the issues and to be able to find ways to improve things. Taking it at your own pace and thinking through things helps.

How was the massage? I really hope that it’s helping with the pain. It’s a good way to help relax the body too. What type of massage do you get?

Good idea about getting your medicines sorted plus having the extra medicine should it be necessary. It’s hard to do that overseas so very good thinking ahead in your behalf.

Im terrible at packing light. I do it but it takes me a while to decide what I’ll really be needing. 10kg isnt a lot. But at least you will be travelling light which helps when you are travelling.

What’s on for you today? Glad the dogs got a good walk!

Re: TW: The 'real' me inside - I come with an SI warning

Hey @Cuddlebear always love hearing from you. 🥰

I started my packing yesterday afternoon - hurray! Hoping I've remembered everything I can pre-pack 😅. But I have a few days to remember things too lol. Half my carry-on luggage is taken up by medication boxes 😆😆 enough for 3 weeks, and i keep them in their boxes (with the pharmacy label) when travelling overseas in case of issues with proving they are mine (never been a problem so far, but just my precaution) - so it take a lot of space that otherwise could be condensed.

I get remedial massages to help release my muscles. I think they are re-tightening less quickly now, but there is still a big "backlog" of tight muscle areas that we work on improving while also keep maintenance on other areas.

Today mostly resting, other than a trip to bunnings and a coffee with my mum. Both of those outings resulted in me feeling quite tired after. I did spend time to paint a sealing coating on the plaster that I painted which I plan to give to my dad for his garden. Good to tick something off my list of to do.

I'm not feeling great today mentally. Not sure if I feel more tired due to it, or if I feel mentally worse because I am tired. Chicken or the egg?

Just the same old thoughts. Wishing I wasnt here anymore, and wishing I wasnt committed to waiting to see how things go next year. But I always keep my promises, and I think that objectively it is reasonable for me to wait until I at least try to see if the 2 things help at all. But its hard when I actually dont want to, Im just dragging my feet making myself exhaust every option. I wish I didnt have to wait 6mths for it. I guess maybe only 4 months or 5 now, but who's counting lol.

I think normally days like today are when I would comfort eat. I'd go to a cafe for an Iced coffee and a treat, or order in something like a pizza, or buy some hot chips. I'm doing well to not do that though, and I'm doing really at getting closer to the target BMI for the trial. Im a bit over half way there. Although on my trip I might not lose any (if the food is really good I migjt gain a little again lol). But im not worried about that, i can enjoy my holiday. But its another reason to not indulge in super-unhealthy or too much comfort eating.

I wish I had something to replace the comfort eating though. I try to distract with TV, but I'm not too engrossed in them usually. Or like now I am so tired I want to close my eyes which I cant do with the foreign ones (which tend to interest me more).

Sorry I'm a bit of a wet blanket today.

How has your day been today?

Give Wilbur some pats and cuddles from me 🥰.

Re: TW: The 'real' me inside - I come with an SI warning

@AlwaysMyself How are you today my beautiful friend.

Great you started the packing and got all the medicines sorted safely for carry on luggage. You’ll be all done before departure.

I believe remedial message is excellent. Everything I have read scientifically tells me it works. You are doing it consistently which brings the best benefits over time.

You are never a wet mop. Ever. I realise how exhausting it is to keep going when we are feeling exhausted from the effort it takes to get through each day. I personally really admire how you do push forward. You find ways to get you through and you try things that can help you have feelings of happiness and joy. You work hard towards those goals and you will get to a point where you do feel better and all your hard work will pay off. I’m here for you. I want to support you in any way I can.

Well done on keeping to your eating plan. And that is awesome your just over walkway there for the BMI you need! You’ve really made good progress here and don’t be too worried about food overseas. Enjoy the holiday and you’ll be ok once you get back with your eating plan. Most of us gain a tiny bit while we holiday.

It’s hard to know what to replace with comfort food. I’m wondering if there is a health comfort food option? Any type of food that is healthy but comforting.

Im still having the tapering battle. My GP wants me to taper more next week. I feel extremely apprehensive about it. I don’t know why he doesn’t understand that this is extremely hard. He’s put me on a new medicine for hypertension and high pulse rate. I started it yesterday and it really added to the awful fatigue I’ve been experiencing. The chemist forgot to put it in my medicine Webster pack for today so I can’t take it today. I get new Webster pack tomorrow. My body is a mess and my brain is a mess and now there’s a new med a beta blocker. I just feel terrified of what it’s all doing to me.

Re: TW: The 'real' me inside - I come with an SI warning

Ack, new meds adding to your fatigue - that sucks. ☹️ is that a common side effect of them? @Cuddlebear

Hopefully just temporary while your body adjusts to lower blood pressure?

As you know I also find fatigue such a hard thing to battle against. Sometimes I've founding sipping at an energy drink throughout the day helps give me that little bit of oomph to still do things - whilst only sipping means that it is a more sustained and not spiked energy source. Thats one of my chronic-fatigue tricks for days I need to keep on going and can't rest. (I'm not a fan of energy drink flavours, but the Solo one actually tastes like a lemon softdrink!)

Is fatgiue the main side effect you're getting from the tapering? Or headache, nausea, etc also?

I assume a doc has done a blood test to check for other things that can also contribute to fatgiue? E.g. Vitamin D, B12, thyroid, iron etc. If not it might be worth it. Although Medicare most often doesnt cover Vit D tests anymore cos they realised most Aussie were deficient and the tests cost them soo much Doctors did them routinely lol. There is a strict criteria for it to be paid by Medicare.

I wish I believed that some day I would feel better / happy / positive / enjoyment. But I don't believe i will. I know there is a *chance* I might, but not a surety. Its been 20 years of trying to do things that will make me feel better. I'm on my last straw of trying.

Part of what is getting me down a little is that if I don't work (in order to maybe be able to live a life I can bear to live longterm), then I won't qualify for government payments. I have an apartment in cant live independently in (and thus rent out), which would disqualify me from any asset test. But the rent from it would not even nearly cover my basic living-with-mother costs, let alone my medical costs. And if I sold it and later recovered enough to live independently, then I'd never be able to afford a house/apartment (even rental) unless i was working 🤷‍♀️.

My mother plans to retire this year, so whilst she has said she could support me living without an income, I don't think she has planned for that in her financial sums. An additional $10k a year would be a lot (the likely shortfall between my rental income from the apartment and the cost of a year of basic living expenses).

Its possible I could ask work about working only 1-2 days per week (Gov job so theyre meant to be adjustable to accommodate disabilities), but I do have concerns about not being informed of updates affecting my work - this was a problem I had when I worked 3 days in the previous team (which im set to return to at the end of the year). Whether working 1-2 days would be suitable is very much dependent on the manager and whether they are organised and allocate the right type of tasks that can be done independently. 😏

I managed to unpack and put away a number of things last night. I think i was doing it for 2 hours before bed?

But the motivational thinking behind doing it isnt a great source. Just preparing for all eventualities next year.
