Managing thoughts of suicide & self-harm

TW: The 'real' me inside - I come with an SI warning

TW: The 'real' me inside - I come with an SI warning

I figure it's better to introduce my challenges here than in a welcome. Hopefully only people willing to hear about SI will venture here.

TW: Suicidal Ideation

Content/trigger warning

I've had thoughts of suicide since I was 12. I don't remember much of life before this time. I'm aware there are times that I haven't though as much about suicide as others, but it's always been a "yes please, that would be my preference" even during the better times.

Why am I still alive then? For two reasons:

- the emotional pain it would cause to very close family.

- the physical pain of accessible methods, and the risks of non-fatal attempts causing lifelong physical impairments.

How do I keep myself going all these years?

By focusing on how I can benefit other people's lives instead. If I'm going to put up with life, it may as well be beneficial to someone.

And to help me keep life less monotonous I look for 'interesting' things - quirky people, exploring foreign countries/cultures, trying new foods (even if i dont think I'll like them). I've never been a status-quo person.

I explain to MH supports that when life is not distressing I can carry on without seriously considering suicide as a possible option. I can say "hello there thought, I hear you but I have chosen not to act on it, things are OK". But when something sets things off kilter, its a pretty quick track back to "why am I still doing this? Do I really still have the same commitment to living? And why, for whom? Why can't I be permitted to die if that's what I want for myself?"

With decades of this under my belt, I have the ability to sit with feelings of extreme distress. And the courage to text or call someone if I need to - and I'm getting a little better at doing this *before* it gets too extreme, but I still hesitate because I don't want to burden others and the frequency of it happening during those unbalanced times is too often. I've persuaded myself a few times recently to instead call a crisis line, but waiting 30-45min in a queue can sometimes exacerbate the thoughts, and sometimes the calls might help dissipate the feelings (if i called early enough) but other times I feel worse after because of course they're going to try to give some positive outlook for the future, but it grates on me because deep down I dont want a future at all. Its actually refreshing when someone acknowledges that not liking/wanting life is OK - and instead just tries to look at how to rebalance the scales to help me cope with continuing to live despite not wanting to.

I'm not sure how un/common my way of thinking is. So much about suicide prevention is focused on short-term distress tolerance or "fixable" problems (eg situations).

What do I need from the community here...?

To be heard? To know if anyone else out there has a long enduring preference to die even when they aren't feeling active distress? And if so, has anything helped them longterm to shift that thinking?

If anyone knows of any practitioners who *specialise* in chronic SI since a young age, I'd really love to know who (if there is a way someone is allowed to??). I've tried Google searching and using online psych directories and reading bio's. I've seen many psychologists and psychiatrists in the past who haven't been able to help me shift this (even if theyve helped in other ways).

I do wonder if my neural networks for positive emotional may not have fully developed or may have atrophied due to the young age and length of time. I genuinely do not know what being happy feels like, I do not have any memory that I recall as being "happy" or joy, and I suspect this makes recovery much harder.

Im ok at the moment. Im safe. But it is exhausting.

Apologies this post is quite a bit too long.

Hey @AlwaysMyself! How’s everything going?
in the residential now hey, how’s that all going? Been thinking of you!
I’ve got heaps I’d love to ask but not sure what I can ask or what you would feel like answering so I’ll throw the imaginary ball to you and give you the freedom to share what you feel comfortable sharing- and no rush.

It is what you expected, how long do you have there?

responding to your other post (sorry for delay🤪)

yess omg the grass seed in Indi’s eye! So so lucky no complications. Eye so fine after but now I have ptsd with where this grass seed was and want to check her third eye lids all the time I don’t but it definitely got past me that grass seed and I don’t like that feeling 🤪

but it didn’t win hehe 😎

You have built such a good team now, you did all the work and you have created a really awesome team. Credit to you my friend, you did so so well!

oh yeas I know what you mean when gps don’t look into things. Treat you for one issue when there is more. My confidence is definitely low with Gps, not sure if I told you but don’t have one atm she left 🤭, but was a good thing!

I have been really surprised that with my upcoming surgery it’s been the anaesthetists who have put in referrals to when they have needed to go 2 seperate referrals to different areas- why? because they actually do an in depth assessment I’d say, had my surgery cancelled once as they wanted something sorted/stablised before surgery so had to wait for that specialist. Although I didn’t like that particular person they gave me the green light for theatre which was the main thing. But do have a heaps of follow ups now. Surgery was done last week so just getting over all that but finally all done yay ✅

take care and hope all is going okay your end 💜

@AlwaysMyself regarding ineligible for hobseeker. answer of ‘due to still owning my own home, my assets are over the limit’ or something similar would have been more than sufficient. All the rest in your answer is TMI.

why do you feel you needed to provide all that info when direct answer would have been less than 30 words?

Because people (generally, not specifically you; I am saying my general life experience) often make incorrect assumptions and thus misunderstand what I say/mean or my context, and then it takes more effort to undo that. Plus myself (and my brain) is a very detailed & accuracy-driven thing, so I guess its normal for me to see & think of all the details and the variety of alternative interpretations a person may make if I only make a short statement. I guess its part of why I work well as an information analyst.

For example (to illustrate), saying “I own a home and that puts me over the limit” is incorrect because owning the home does not put me over the asset limit since the home you live in does not count towards the asset test. 😊 The defining feature is that I don't live in the home, but it's due to my disabiltity that I don't currently. And this is a distinction between the asset test eligibility for JS vs DSP too.

I hope this helps gives some understanding of why I often feel “too brief” causes people to misunderstand me or the problem/issue. 😊

Re: Low income card - when my work paid leave ends, yes I could. But in the interim between then and when my DSP is assessed/approved I won’t have any income, even if I do get a low-income healthcare card in the gap time. But I will reach/exceed the Extended Safety Net threshold before that time anyway, so it will then be of little benefit as far as I am aware (since I will get an 80% gap fee rebate from medicare). But thank you for the suggestion, i do appreciate it. 😊

‘right person, right timing’ can definitely make a difference.

I believe there are lot of good people out there working in the broad ‘medical field’.

we don’t always get to work with the ‘right person’ ie some one who we easily have rapport with, some one who actually listens and hears.

it can become very difficult and challenging to try some one new after having seen xx in the past who were not the ‘right person’

100% agree. 😊

Re: residential program, yes, I didn’t want to say too much about it whilst I was there. i may share a little more about it now I am out (in a subsequent post) — it was very mixed experiences, some healthy and some harmful to my attempts to grow.

I managed to get appointment with my GP this morning (was originally booked for mid May). GP happy with sound of my lungs, has approved for me to start having B12 injections (first one booked for 2 days time) plus agreed ultrasound on right shoulder is best next step. Outcome of ultrasound could be injection into the joint or different scans. My small amount of ‘me time’ for next couple weeks has just gotten even smaller. Am booked in for flu shot end of the week (there goes my plans for this weekend), need to talk to nurse at GP on day of flu shot about dates for my Covid boosters this year.

Such good news about getting into the GP early and their clearance of your lungs and the B12 injections. 😊 So glad!

I hope the ultrasound goes well and can give some insights into what is happening with your shoulder, and how to best treat it. 💜

Fingers crossed any negative-reaction to the flu shot will be minimal. Is it common for yourself that your body has a reaction? (I feel very fortunate that my body doesn’t seem to get flu symptoms after either flu shots or covid shots, but I know a number of people who do. It sounds like you are also in that case, but I don’t want to make an assumption.)

Re: trial - yes, this week was the first dose. I will do a separate post, but I don’t think i will go into much detail of it (on my standards, at least lol). But the dosing day went very smoothly and positive experience.

I had to be discharged from the residential program prior to the clinical trial starting (to avoid inference of “medical complications” which are not related to the trial treatment), so the residential managed to get me in just in time to complete their program before my dosing day 😍. I was very lucky and grateful they could do that at short notice from referral.

@AlwaysMyself for some reason life feels so boring for me right now. The thought of having to do the same things in my daily life everyday makes me think I should do some other things in my life that can REALLY keep me occupied during the day but it would’ve been better for me to easily pursue that if I had a lot of support people in my life AND if i wasn’t neurodivergent & didn’t had to experience the issues and struggles I’ve been having since at a really young age.

Also, I know you always like to give problem solving suggestions but please know that not everyone including myself will benefit from having the concerns we share trying to be ‘solved’. I know Flutterbug9 has expressed the same thing last time so I’d like to also let you know about this as well.

I feel you about the monotony of life at times. Sending a hug. Also on the challenges/barriers of being able to pursue what you really want to. I’m sorry you have had to (and continue to) face these feelings also. I wish there was something I could do to help ease the pain.

I appreciate (and take on board) the feedback about problem-solving - thank you. 😊. I will deliberately refrain from making suggestions to you as much as possible, unless explicitly asked for advice. I guess part of my struggle is knowing what/how support does look like for you, from how I can? Is it just listening and acknowledging the pain?

I want to support you in the way that is best for you; I just don’t always know how (without asking) since everyone is individual in that. 🤗

Delays are always OK - i have been the queen of delayed reading & replying myself lately too.

So glad there were no complications 😍 - what a relief! Hearing you on the fear of it happening again though. 🤗 It can be such an uncomfortable feeling, and I hope it fades in time as your nervous system resets. Sounds like you are mentally handling it well though with the self-talk and understanding what is going on for your brain/nervous-system with the anxiety?

Very proud of you that you aren’t letting the anxiety win your mind. 😍😎 *high five or equivalent gesture*

Oh, I don’t know/recall whether you mentioned you GP left. I think so - it does ring a vague bell in my mind. But then I’m also not sure if it could have been someone else instead tell me a similar thing as I don’t recall who! 😖 Ugh my brain memory is not what it used to be - sorry! I hope you can find a good new GP soonish.

I am so glad the anaesthetists are really thorough though! I guess they probably are acutely aware of how many things can influence sedations/anaesthetics and how dangerous it can be to get it wrong, so from a risk perspective it might be drilled into them more?? I’m sorry they had to postpone the surgery temporarily - but I hope it was a feeling of “better safe than sorry” for you and the delay wasn’t a significant stress/impact for you?

Glad you got the green-light (even if the specialists wasn't the most comfortable fit/experience) and the surgery went ahead 😊. I hope you’re going ok (or better than) in the post-op recovery, and hope that the nurses & specialists are all very supportive — especially when they know you don’t have a GP currently to hand over community-based management of it to. (Always open to talking about it, but don’t want to pry or be perceived as pressure so I’m not asking questions - please don’t take this as disinterest or lack of care though 🤗).

@AlwaysMyself thankyou sweet, things have been incredibly tough for me lately. I'd say that I'm doing my best but then I don't feel like my best is ever good enough. I'm safe for now, going to try to go back to sleep. I need to clean for a house inspection but I have nothing in me right now.

Re: TW: The 'real' me inside - I come with an SI warning

@AlwaysMyself This is an aspect of where our minds work differently. Because topic of conversation was only Jobseeker, all the info about anything else my mind reads as not relevant and TMI.

I understand about providing lots of info at times to try to avoid confusion and misunderstanding. Due to outcome of DSP not being known, it didn’t make sense to my mind why the mention of it. All good.

Info on, tpwhat appears to me to be, unrelated topic causes confusion. 10 days in constant pain in dominant shoulder joint, nightmares, disturbed sleep and not being able to do any type of self care etc is having huge negative impact on me. Dealing with emotional dysregulation and nothing I can do to help. Can’t even tolerate to use any fidget toy and haven’t for days

differences between our line of thinking, plus not disclosing personal info, my answer might have been about assets over the limit for jobseeker, higher asset limit for dsp.

medicare gap rebate limit - does that allow you to get cheaper scripts? Are cheaper scripts only available if have a health care card?

only reaction I tend to get with flu and covid injections is reaction at needle entry point. I get large, raised red patch that is hot to touch. Fairly common for me to be at GPs rooms 7 to 10 days after injection to either get script for meds and/or given needle to counteract the reaction. With flu x 1 and covid x 2 each yr, it’s become a standard routine

ultrasound Monday, Friday before I get results. GP thinks might be bursitis flareup. Has said if that’s isn’t the cause I’ll need to have X-rays and scans of shoulder prior to referral to specialist, also possible shoulder surgery which I’m hoping never to have to go through

good to hear your first day in the trial was a positive experience for you. Sorry to hear you have had some negative experiences recently.

Re: TW: The 'real' me inside - I come with an SI warning

A few people have asked about the residential program I was in the past few weeks, so I will share a little more.

@Chasingsunsets @Flutterbug9 @Cuddlebear

I think it’s similar/equivalent to what I have read about the Victorian APARC programs? Except that I wouldn’t say the support workers were very well trained in mental health (some were quite well trained, others very minimally and insufficiently - like they had maybe done a 2-day training course and it was more rote-learnt for them rather than intuitive or deeply integrated into their understanding), and with very, very minimal 1:1 support available (despite it being stated otherwise).

The first week was a positive experience. The second week was not.

Part of the second week’s experience was of limited-control by the staff (another resident’s behaviours). But the hurtful (and psych-harmful) part of it was directly a result of staff interactions 😓, and unfortunately reinforced many reasons why I don’t seek help or support from people — which was actually one of my goals for the program was to learn safely to be able to ask for support or self-advocate for accommodations I need (eg physical disability adjustments). I feel the staff incident has been left unresolved, and not yet decided whether to follow-up with the managers. (If interested, read back in my last post to AuntGlow where I mentioned it.)

I did meet a few other people there that were positive experiences though - both a few staff and a few peers. And I can keep in contact with a few of the peers too, which adds to social connection for me. 😊. That is a positive outcome, and one I will try to focus on.

But I do also want to encourage that I think most people find the service overall helpful and supportive, and it more depends on what your individual needs are and the staff who happen to be on at the time. Such variability.

Would I attend again in the future if I needed extra support? Maybe. But only with a very in-depth intake discussion and detailed instructions/plans and adjustments agreement with the staff to ensure that the same problems did not reoccur, and how I could be supported to assert the support-needs if staff did not uphold it (or forget to). I don’t want people to think I am discouraging people from the program - I’m not. It has its place and can be positive.

Re: TW: The 'real' me inside - I come with an SI warning

Glad you are taking rest and being gentle on yourself today @MissGremlin . Big safe hug (perhaps via Luna?).

Its so hard when feeling that your best still isn’t enough and still struggling. 😓. I wonder/hope whether there is anyone you can ask and trust for help to bridge that gap even just for this weekend. One step at a time. One day at a time. I believe it’s OK to only focus on the true essentials for survival and let everything else go for a short bit. 💜. Maybe that is “enough” - just food and sleep?
