Managing thoughts of suicide & self-harm

TW: The 'real' me inside - I come with an SI warning

TW: The 'real' me inside - I come with an SI warning

I figure it's better to introduce my challenges here than in a welcome. Hopefully only people willing to hear about SI will venture here.

TW: Suicidal Ideation

Content/trigger warning

I've had thoughts of suicide since I was 12. I don't remember much of life before this time. I'm aware there are times that I haven't though as much about suicide as others, but it's always been a "yes please, that would be my preference" even during the better times.

Why am I still alive then? For two reasons:

- the emotional pain it would cause to very close family.

- the physical pain of accessible methods, and the risks of non-fatal attempts causing lifelong physical impairments.

How do I keep myself going all these years?

By focusing on how I can benefit other people's lives instead. If I'm going to put up with life, it may as well be beneficial to someone.

And to help me keep life less monotonous I look for 'interesting' things - quirky people, exploring foreign countries/cultures, trying new foods (even if i dont think I'll like them). I've never been a status-quo person.

I explain to MH supports that when life is not distressing I can carry on without seriously considering suicide as a possible option. I can say "hello there thought, I hear you but I have chosen not to act on it, things are OK". But when something sets things off kilter, its a pretty quick track back to "why am I still doing this? Do I really still have the same commitment to living? And why, for whom? Why can't I be permitted to die if that's what I want for myself?"

With decades of this under my belt, I have the ability to sit with feelings of extreme distress. And the courage to text or call someone if I need to - and I'm getting a little better at doing this *before* it gets too extreme, but I still hesitate because I don't want to burden others and the frequency of it happening during those unbalanced times is too often. I've persuaded myself a few times recently to instead call a crisis line, but waiting 30-45min in a queue can sometimes exacerbate the thoughts, and sometimes the calls might help dissipate the feelings (if i called early enough) but other times I feel worse after because of course they're going to try to give some positive outlook for the future, but it grates on me because deep down I dont want a future at all. Its actually refreshing when someone acknowledges that not liking/wanting life is OK - and instead just tries to look at how to rebalance the scales to help me cope with continuing to live despite not wanting to.

I'm not sure how un/common my way of thinking is. So much about suicide prevention is focused on short-term distress tolerance or "fixable" problems (eg situations).

What do I need from the community here...?

To be heard? To know if anyone else out there has a long enduring preference to die even when they aren't feeling active distress? And if so, has anything helped them longterm to shift that thinking?

If anyone knows of any practitioners who *specialise* in chronic SI since a young age, I'd really love to know who (if there is a way someone is allowed to??). I've tried Google searching and using online psych directories and reading bio's. I've seen many psychologists and psychiatrists in the past who haven't been able to help me shift this (even if theyve helped in other ways).

I do wonder if my neural networks for positive emotional may not have fully developed or may have atrophied due to the young age and length of time. I genuinely do not know what being happy feels like, I do not have any memory that I recall as being "happy" or joy, and I suspect this makes recovery much harder.

Im ok at the moment. Im safe. But it is exhausting.

Apologies this post is quite a bit too long.

@AlwaysMyself Were you given reason/s why you can’t get jobseeker money?

No probs with you asking questions.

Im happy to share knowledge and/or life experience/s with you.

exception to this would be if I’m not comfortable answering.

if that happens, I would let you know

Main topic of discussions with my psychologist recently has been autism and the different way our brains work @AuntGlow

Earlier this year I told my psychologist of my goals which we will go back to at some stage. I’m starting to see where autism connects into some of the struggles behind my goals and my not understand some of my decisions/reactions last year.

i will definitely be continuing the discussions with the psychologist and hoping to discuss with my new psychiatrist, initial appointment in June. My psychologist has already offered to write letter to my psychiatrist. First time in my 60+ yrs of life I can remember this much support and understanding

Hello lovely! I am catching up on notifications and working backwards, so I will see your more recent updates soon later, but wanting to address this first. @Flutterbug9 🫶

I can imagine those conversations with your therapist are really helpful and enlightening?

I wonder what would help your brain feel more supported to in order to reach your personal goals? Learning what works for neurodivergent brains is so empowering. 🥰

You absolutely deserve this kind of support, I am so happy to read this.

Helloooo, my friend! I am reading through some of your updates from earlier this month now - I am working backwards, so I understand you may have further updates, but I will touch on this one first 💛:

Update from my side...

- second half of week went mentally-well, but my blood pressure symptoms have continued to be very concerning to the nurses so i have been tracking it (when i remember! Consistnecy of time of day isnt easy) so that i can discuss temporary medication for it with my doctor. It is something i have previously noted to the doctors but it had only been "during acute crisis", not the prolonged post-crisis hypertension which has occured this week. It appears i have been rather lucky to not have had a stroke a few times this past week 🥲, according to the nurses based off my symptoms & pressures. That was not easy news to hear a few days ago, since I have already been trying to do everything i can to calm/breath/etc to reduce it, and the only other option is to call an ambulance / go to ED which I have a strong mental resistance to. But i have agreed i can call an ambulance (who cant force me to go to ED unless im incapacitated) next time it happens that severely. I guess this is a different type of "safety plan" to what is usually required for mental distress 😅😓. I just feel so stupid/wasteful when its due to emotional distress, but a stroke would be very serious.

- pdoc's proposed plan for me for April (until we start the med dosage) was accepted by the referred-to provider. This should help give me some lower-stress respite plus extra non-clinical support prior to the Trial. I can't go to a hospital (unless absolutely required) because it would impact the research data (and I dont want something unrelated to the research to do that), so this is our alternative plan to avoid hospitalisation.

- i need to figure out how i need to best grieve the confronting history of missed-diagnosis, dismissals, and (in one case) a pdoc not believing me in the past. My inner justice wants them to be aware of their past mistakes (in case they are not since aware, to provide awareness for future patients). My hurt would like (but not need) acknowledgement and maybe a genuine apology (when dismissed/failed/disbelieved). But I also know most people do not take painful feedback well, and some people get defensive-aggressive in response. I'm also aware a complete lack of response would be painful and reinforce the past experiences of dismissal. I'm not yet sure the risk-benefit of it, especially for historical providers (but they were the pdocs who should have noticed something wasnt normal and investigated further). The number of incorrect representations in reports/letters/etc is painful.

- i was able to speak to my physical-health GP briefly after we completed the paperwork, about how severe i was when i left his office last week. I was able to give him to read something i had written the day prior to my seeing him, which would have been very confronting about my state of mind, as it had explicitly stated my distress-triggers and what i think when triggered. I was able to then tell him that was "before i had the meltdown in his office" and explain that if i wasnt mentally-incapable of breaking the promise to my pdoc (which most peeople dont have to my degree) that i wouldnt have still been alive when he called 2 days later. Whilst he didnt say much, i think he understood and accepted the seriousness and i have hope that it might stick in his mind for the future with other patients. But hoenstly i dont think he knows how to react or what to say/do when he sees someone with those thoughts; in hindsight i wish he had asked what he could have said/done (or that i had offered him to ask me if he is ever unsure). I guess i don't experience the "freeze" when i hear a person in distress, so i don't really know what that is like for others.

Im falling asleep typing this, so that must be enough for now.

Weekend has been emotionally and socially challenging with my visitor. They go home tomorrow evening. May talk about it more another time.

No expectation to respond. Just tagging as sometimes you like to know updates and i dont know when i'll be able to get much forum time.

Oh my goodness, blood pressure is no joke. Mine goes up and down, and it can be really anxiety-inducing. I am so relieved you're okay and I think you are handling your health really well by keeping in touch with your doctors. Do you have any updates around this? @AlwaysMyself 🫶

Around the topic of grief, it really is so understandable that you would be experiencing a sense of being let down by your health professionals. I think wanting an apology and wanting to raise awareness is valid, but I also get how a lack of response would be quite triggering... where did you land on this? I would absolutely support whatever decision you decide to make. 🥰

I understand your feelings around your GP; it can be even more triggering to be met with someone who can't co-regulate with you or reflect back what they are hearing with attunement... I wonder if you could give him that feedback? Part of any health professional's role is to continuously learn and develop their skills, and in my opinion, that absolutely includes being able to soothe a patient.

I am looking forward to more updates, and I will get back to you as soon as I can. I am going on leave next week for the show I am doing, so I won't be here, but I will be back after then! 🫶

Re: TW: The 'real' me inside - I come with an SI warnin

One of the goals I mentioned to my psychologist I’m wanting to explore as I didn’t understand. @AuntGlow feeling genuinely excited about going to something and within short period of time start to feel anxious and loose all interest in wanting to go.

eg last year day time cruise for few hrs was organised by a person in the village where I live. Day 1 I felt all excited and was looking forward to going. By day 2 I felt the complete opposite.

Social Avoidance and Isolation, the combination makes so much sense. I now have the “why” which has helped some. The struggles and inner turmoil still happen, I find knowing The Why processing the contradictions does help.

my psychologist has said in their experience, a very common theme that comes through from neurodivergent people is “Why”.

having a therapist who fully supports and openly talks about impact of being neurodivergent as compared to one who verbally shut the door on any possible discussion (my previous therapist) is a huge positive difference

@Flutterbug9 re: jobseeker ineligibility

I would have to apply for Finacial Hardship provisions (which takes as long as reviewing my actual DSP claim) because of my apartment value being over the limit.

Because I dont live in my apartment anymore (because i cannot live independently without very significant supports, and i cant financially do that unless i get significant NDIS funding), and thus rent out my apartment, So that means my apartment counts for the Asset Test, which means i exceed the asset test.

But the hope is i can get the supports i need to live independently, so selling it is also not a real longterm idea either.

So under DSP there is a higher Asset Test limit, and my apartment is under that limit.

And if i move back into the apartment it wont count for *either* JS or DSP asset tests.

*edited later to remove some details*

@AuntGlow i hope your show goes well 🥰 and glad you can take leave to fully focus on it and enjoy it too i hope! 😍 example of a balanced life and selfcare i think.

Blood pressure update. Been on medication for it for abit over 2 weeks now, so it should start to be kicking in. Starting on a low dose since we dont know how much or little my body will respond it to, and of course dont want to drop BP to low BP levels when i'm OK/calm.

I think its started to help a little, but its hard to know. My calm moments do seem up to 10pts lower (which is good), and when j had a major meltdown last week i didnt measure my BP (i didnt think to haha) but i also didnt get the severe BP pain symtoms to the level i was feeling when i got into clinical urgency/emergency stages - so i think that is a really good indicator.

The real "test" might be this week's medication dosing day. They will monitor my BP for the first 3 hours they said, so they will get a good read on how it sits whether i am having a calm experience or a distressed or excited/hyper experience 😆. And hopefully i can also take that overall picture/advice back to my GP next week when we review if to increase the dose. I suspect we will step the dosage up a stage though, based on my current BP readings.

Re: GP and other doctors feedback & grief processing.

I haven't decided yet. But i think i will get in contact with an organisation that has been recommended to me to talk it over with them, get some advice (and support in writing it if i chose to).

I had a bad experience last week where i am staying, and as part of that the managers said they would speak to the staff member and suggest they speak to me about their perspective of what happened (as it would help me) but when i next saw that staff member they didnt - and that has made it harder for me *because* i know they were encouraged to talk to me about it and chose not to (despite available time/space and me being friendly still to show non-threat). 😥. Its just another emotional response i now have to process on top of the initial incident (which i will note the managers said i did the right things and it sounded like the staff could/should have done different to what they did and they'd look into it with the staff for the future improvement. I'm not being over-reacting in my expectations of what they could have done; i am being realistic and taking their constraints into account.).

It feels like a setback each time, and makes the learning-to-self-advocate or ask for help harder when i have these experiences again. They're part of the reasons i learned to not ask for help or self-advocate.

Instead i do try to focus and recall the people who i have had positive experiences with and remember that some people are safe and "right for me" -- but then its learning how to predict and assess these people so i dont open myself to the other ones who aren't. Because "being a professional in a support/medical role" is obviously no guarantee. 😥 and sadly i think too many others here have also exprienced that reality too.

So much of life is currently "up in the air". Which doesnt do well for liking predictability 😅.

Today is my last full day in this program, then back home tomorrow. Mixed feelings.

Take care and likely 'see' you when you're bavk from leave. 🥰

Re: TW: The 'real' me inside - I come with an SI warnin

@Flutterbug9 i relate a lot to what you said about understanding the "why" and the mix between wanting to do and not wanting to do.

*hugs* you're not alone in it.

I am still so glad and relieved your new therapist is so affirming, supportive, listening, experienced/understanding, etc etc. 🥰😍🤩.

They sound so great, and you absolutelt deserve to have someone who understands and supports and accepts/celebrates who *you* are genuinely. 🥰. What a world of difference it can make.

I also love hearing about it (in addition to the very real benefits for you) because it also helps to remind me that there are people/professionals out there like that; to balance out the negative experiences from some others too. Thank you.

@AlwaysMyself regarding ineligible for hobseeker. answer of ‘due to still owning my own home, my assets are over the limit’ or something similar would have been more than sufficient. All the rest in your answer is TMI.

why do you feel you needed to provide all that info when direct answer would have been less than 30 words?

could enquiring about Low Income Card be an option?

https://www.servicesaustralia.gov.au/low-income-health-care-card

Payment finder can be helpful at times

‘right person, right timing’ can definitely make a difference.

I believe there are lot of good people out there working in the broad ‘medical field’.

we don’t always get to work with the ‘right person’ ie some one who we easily have rapport with, some one who actually listens and hears.

it can become very difficult and challenging to try some one new after having seen xx in the past who were not the ‘right person’

I just skimmed through your post to AuntGlow, something about a program you are/have been doing and moving back home soon. I’m hoping the program has been beneficial for you and you have gotten positives out of the program.

I managed to get appointment with my GP this morning (was originally booked for mid May). GP happy with sound of my lungs, has approved for me to start having B12 injections (first one booked for 2 days time) plus agreed ultrasound on right shoulder is best next step. Outcome of ultrasound could be injection into the joint or different scans. My small amount of ‘me time’ for next couple weeks has just gotten even smaller. Am booked in for flu shot end of the week (there goes my plans for this weekend), need to talk to nurse at GP on day of flu shot about dates for my Covid boosters this year.

is it this week (week starting Mon 27 April) that you receiving the first med as part of the trial happens? Will the trial still start this week or will it be deferred due to this program you are currently participating in? (Unsure of appropriate wording to use as have zero info about the type of program)

Re: TW: The 'real' me inside - I come with an SI warning

@AlwaysMyself for some reason life feels so boring for me right now. The thought of having to do the same things in my daily life everyday makes me think I should do some other things in my life that can REALLY keep me occupied during the day but it would’ve been better for me to easily pursue that if I had a lot of support people in my life AND if i wasn’t neurodivergent & didn’t had to experience the issues and struggles I’ve been having since at a really young age.

Also, I know you always like to give problem solving suggestions but please know that not everyone including myself will benefit from having the concerns we share trying to be ‘solved’. I know Flutterbug9 has expressed the same thing last time so I’d like to also let you know about this as well.
