Managing thoughts of suicide & self-harm

TW: The 'real' me inside - I come with an SI warning

TW: The 'real' me inside - I come with an SI warning

I figure it's better to introduce my challenges here than in a welcome. Hopefully only people willing to hear about SI will venture here.

TW: Suicidal Ideation

Content/trigger warning

I've had thoughts of suicide since I was 12. I don't remember much of life before this time. I'm aware there are times that I haven't though as much about suicide as others, but it's always been a "yes please, that would be my preference" even during the better times.

Why am I still alive then? For two reasons:

- the emotional pain it would cause to very close family.

- the physical pain of accessible methods, and the risks of non-fatal attempts causing lifelong physical impairments.

How do I keep myself going all these years?

By focusing on how I can benefit other people's lives instead. If I'm going to put up with life, it may as well be beneficial to someone.

And to help me keep life less monotonous I look for 'interesting' things - quirky people, exploring foreign countries/cultures, trying new foods (even if i dont think I'll like them). I've never been a status-quo person.

I explain to MH supports that when life is not distressing I can carry on without seriously considering suicide as a possible option. I can say "hello there thought, I hear you but I have chosen not to act on it, things are OK". But when something sets things off kilter, its a pretty quick track back to "why am I still doing this? Do I really still have the same commitment to living? And why, for whom? Why can't I be permitted to die if that's what I want for myself?"

With decades of this under my belt, I have the ability to sit with feelings of extreme distress. And the courage to text or call someone if I need to - and I'm getting a little better at doing this *before* it gets too extreme, but I still hesitate because I don't want to burden others and the frequency of it happening during those unbalanced times is too often. I've persuaded myself a few times recently to instead call a crisis line, but waiting 30-45min in a queue can sometimes exacerbate the thoughts, and sometimes the calls might help dissipate the feelings (if i called early enough) but other times I feel worse after because of course they're going to try to give some positive outlook for the future, but it grates on me because deep down I dont want a future at all. Its actually refreshing when someone acknowledges that not liking/wanting life is OK - and instead just tries to look at how to rebalance the scales to help me cope with continuing to live despite not wanting to.

I'm not sure how un/common my way of thinking is. So much about suicide prevention is focused on short-term distress tolerance or "fixable" problems (eg situations).

What do I need from the community here...?

To be heard? To know if anyone else out there has a long enduring preference to die even when they aren't feeling active distress? And if so, has anything helped them longterm to shift that thinking?

If anyone knows of any practitioners who *specialise* in chronic SI since a young age, I'd really love to know who (if there is a way someone is allowed to??). I've tried Google searching and using online psych directories and reading bio's. I've seen many psychologists and psychiatrists in the past who haven't been able to help me shift this (even if theyve helped in other ways).

I do wonder if my neural networks for positive emotional may not have fully developed or may have atrophied due to the young age and length of time. I genuinely do not know what being happy feels like, I do not have any memory that I recall as being "happy" or joy, and I suspect this makes recovery much harder.

Im ok at the moment. Im safe. But it is exhausting.

Apologies this post is quite a bit too long.

@Till23 so glad you got into the trial and MH wasn't a barrier. Would love to hear how you with it *if you wish to share* (never any pressuere to share. But *always* permission to share no matrer if its positive or negative).

Sorry to hear your recent medications didnt help the mood imrpove ☹️. Your description of it sounds really similar to mine.

I've always said i "dont have/feel anxiety" because i felt so dead instead and didnt have any motivation to care enough to feel anxious -- except about running late to things! (Which i managed by never running late lol !) But coming off the medication thisnyear I've noticed feelings similar to feeling of anxiety i felt at the very tailend of when i had my panic attacks over a year ago (so the mild nearly-gone anxiety feelings; not currently getting any problematic anxiety other than preoccupied thoughts that are emotionally depairing but realistic).

@AuntGlow i also meant to ask how your recent changes have gone meds-wise? (If you wish to share. If not, then "I hope it is going well / a good change".)

It’s interesting to hear that the book brought up so much emotion for you, where are you at now? Do you feel adhd is a possibly of something you have?

The book the year I met my brain was my light bulb moment for my eventual diagnosis of adhd. Nearly a year diagnosed now. It was pretty funny my work friend was reading a book at lunchtime and it had colours on it and I saw the word brain on it, so I asked her what she was reading. She said she had been just diagnosed with adhd and would I like to have a read. I didn’t know much about adhd only superficial knowledge and just about the hyperactive side. While quietly flicking through I was like ✅✅✅.
this started my research and journey to a possible diagnosis which from referral to diagnosis just over 8 months.

I think there is definite sadness with diagnosis’s that happen later on in our lives. To be honest I’m not sure I have fully come to terms with it myself. How did all these people miss this. From a young age couldn’t spell behind in everything, tried really hard still got nowhere for my whole schooling days. I always knew I was different but I didn’t know why. Struggle with processing auditory and visual my whole life, no real good executive functioning, frequently misunderstood, MH from 15years old ish- counsellors/psychs and Pdocs Gps, just a lot. Since when does the ‘patient’ diagnose themselves ?? Meant to be the professionals right? But yes not always the case as we have been seeing. Very very interesting.

I hear the time away was tough I am glad there were some good parts.

You sound super busy with all your admin on getting everything together, good on you though, it’s a big effort and I am sure it is overwhelming at times but you are doing so well 💚

Keen to hear @AuntGlow too 🙏, happy to be tagged only if you want of course ☀️

Re: TW: The 'real' me inside - I come with an SI warnng

@Chasingsunsets I love blankets and pillows/cushions - so you are absolutely allowed to put one on your emotional surfboard!

Actually, I was thinking the other day “What makes me comfortable?” and realised having one of my cushions to hug and/or a light blanket over me makes me feel much more at ease, and I will probably ask (or just do - I don’t think I actually need to ask) to do this for my PAT sessions (if I remember to!). I think it would help put me at ease more, not that I generally have a feeling of anxiety/unease. Or well, I didn’t until I came off my medication the past 2 months… 🤣 I “warm up” quickly to people though, so it’s not a problem if I don’t.

I realise I might be taking this question too literally (😂) but I don’t really know how else to answer it lol.

Re: book "where are you at now?"

Because your mentioning it reminded me I had been listening to it, I have again started to listen to it (when winding down for bed, whilst doing laundry…) and I’m up to the chapter where she talk about the internet/social-media algorithms predicting/presenting ADHD content. PS - as someone who has worked in web SEO & SM marketing a little it makes absolute sense to me (it feels “obvious” to me, because of my past work experience/learning) that people are presented with ADHD content by SM algorithms — for multiple reasons (anyone who is interested in why, please ask! I don’t want to info-dump without being asked lol).

As for if ADHD is something I could possibly have… I don’t like to “get my hopes up” of being validated (I.e. Yes, very much feel it is a strong possibility but I find it hard to sit with that when I don’t know yet) in case I don’t. That’s why it was so emotional listening to it at the start of the year - internal battle & dysregulation.

Keep in mind, I am currently having an Autism & ADHD (and other MH) assessment because my new GP is very sure I am autistic and has also asked if I could be ADHD. Since mentioning the autism assessment referral to other MH-supports since then, I have also had my counsellor and the PAT psychiatrist ask if I have also considered ADHD (and not just due to the comorbidity rates, due to other struggles). The MH peer worker I’ve seen about 4 or 5 times now said she will be surprised if I don’t receive a diagnosis of ASD+ADHD (hearing this helps - maybe I’m not just over-identifying?) because of her own observations & discussions with me.

...so… yeah? But not “just ADHD” - AuDHD.

I didn’t realise you had an adhd diagnosis 🤗. I won’t lie.. you know the social phenomena of unintentually gravitating/selecting friendships of other ND people? I don’t think I have even one friend that I feel I truely “click” with that does not identify/diagnosed or strongly display (even if in denial lol) either ADHD or ASD traits. I think all of them identify/diagnosed or have significant traits (which would warrant a referral for a screener) that are consistent with ADHD. Oh actually, maybe two friends with significant high-masking autism maybe but I don’t know them as well to have had discussions around ADHD or not. Neither have sought diagnosis, but both have children with significant ASD and ADHD 😂.

I say this because… well… I’ve always felt I “clicked” with you too? (along with a number of others here too, of course!)

So… it kinda also holds to this pattern? 😅

The psychiatrist for my trial specialises in ADHD in his private practice, and he is a rare-breed that accepts ADHD diagnosis from a psychologist without needing to do his own assessment (just normal extended initial consult) for prescribing medications & management-advice. So if the psychologist does diagnose ADHD for me, then I can book an appointment with him privately once my trial has ended if I wanted to consider medication. Plus one of the 2 possible medications in the trial that is given (single-dosage-days) is an ADHD medication. So that will be really interesting to reflect upon after being “unblinded” at the end of this year, if I received any doses of that medication.

"I think there is definite sadness with diagnosis’s that happen later on in our lives... How did all these people miss this."

I don’t recall if I have expressed this myself on the forum already or not, but if not - 1000% yes!

I said to a friend today that it is “kind of validating, but also kind of sad that I was always made to feel I had to keep pushing through and none of this was ever investigated further”. It’s a good thing I don’t have any paranoia, or else I’d think you were reading my phone messages earlier today 🤣… even though you posted this yesterday night lol !

but yes, 1000%, 10000% even! Thank you!

I have to go out now, but I really really want to respond more to what you have said - and I will! SO many similarities in what you just posted. And I want you to know you are not alone, it is not your fault, and it so good that finally things are being “realised” by the doctors too (but also sad its also taken so long for you also) — and that for me talking about this is “recreational” and uplifting (but that the problem - if i’m not careful I will spend hours doing it and nothing else! Which is why I have alarms to tell me to stop and needing to “time block” lol. See.. ADHD much? I just… behaviourally manage it, right?)

Another topic I’d love to chat about if anyone else is interested - but I don’t have time now, so just plopping it here.

I’ve been reading a bit about “nervous laughter” and “inappropriate laughter”. I don’t know how to know what is “normal” and what is “more than normal”.

But in this reading, I also came across this blog post. Which in case anyone is curious how I think, I felt was 100% “me” on all 14 statements lol. Enjoy! https://listickle.co/people-who-are-never-embarrassed-by-anything-often-do-these-things/

“I don’t get embarrassed. I laugh about it!"

(…same as nervous… and it’s also how I try to reduce anger too… rationalise & laugh!)

Hello lovelies! Happy Saturday. @AlwaysMyself @Chasingsunsets

Re: medication. I have only just started shifting to a lower dose of my medication, so it's still early days! I will have to keep you posted, if that's okay. 🫶

@AlwaysMyself ahhh, I wish you could share more, but I understand why you might not be able to. 💛

Sooo exciting! I am really, really keen to hear what comes up for you and how your body/mind responds to the treatment.

Gah, the physical symptoms of adrenaline really would feel like a lot. Does anything help you to regulate in those moments?

It's so interesting how medications impact people differently. It goes to show that there isn't a real right or wrong, but more so, what works for you and where you're at.

Oof, finding 25 year old history would be laborious to say the least! How much information do they require?

I hope you're feeling okay today and that you're doing something nourishing for yourself this weekend. ✨

Best wishes for the adjustments @AuntGlow - and please feel most welcome to also share here about it in any way that also helps you. 😊 (I.e. I'm OK for you to step out of the “what helps the other person?” role for a bit and also post about what might help you or feel helpful to be able to “tell a peer” even if it doesn’t directly relate to what we’re already chatting about . Open permission from me. 🤗)

well… and it’s OK to laugh, I do… it feels kinda hyperactive? so…. fiddling (or in my case, rubbing/itching my arms) 🤣… or when I’m standing - either pacing or rocking on the sides of my feet 🤣 …

but since they can sometimes make it hard to do something like say “typing on a keyboard” that’s where I end up putting on some music (something I’m familiar with and don’t need to use any focus on, but “feels right”) - sometimes this can be metalcore, sometimes it’s an acoustic version of a metal song, and sometimes it’s a bit more of a general “rock” with a bit of a lighter/faster tone until I am feeling more calm and move to a metal/core song instead lol.

… see why I am now wondering if yeah … maybe my medication was dampening a lot of these adhd-like jitters?
because I’m not worried about anything! I am competent. But I have a thousand things in my mind and on my list to do - which I always have had, but since coming off the medication I feel more hyped up (energy) despite the same thought patterns? (I was on an SNRI, so there was a noradrenaline component - for pain and fatigue indications.) 😬

I'm actually really glad that I’m off my medication for the ASD/ADHD/MH assessment because I can see the difference and give them “both pictures” now - which might help them figure out what’s going on?

Oof, finding 25 year old history would be laborious to say the least! How much information do they require?

Yeah thats the problem. I have no idea! I mean, “treatment history and outcomes” for each condition.

But if I don’t give enough detail they can reject it and it can prolong the process by many months to have to re-submit for review with extra info/details.

So I have been advised to “give everything” and let centrelink & DSP get the “full picture” of overwhelming evidence for themselves to decide.

My main concern is… Will my MH GP have enough time (appointment availabilities) to go through the paperwork with me? (many psych records aren’t released to me of course - so they have to go to her for her to filter through)

….and... does she want to? is she willing to do it when it may be so much time/appointments and she is so busy with a waitlist for new appointments? I know this will be a “once off” massive effort, but is SHE ok with that?

Of course I’m paying full-fee and offering to pay for any non-face-to-face-time also, so I don’t feel like that is a pressure I am putting on her that she has to do unpaid time.

I will be asking her / checking with her when I see her on Tuesday. But I did say at the first & last appointment that my reason for coming to her to be my regular MH GP was because I need more support and applications for DSP & NDIS because my existing general GP has no interest in MH (the stories I could tell! people are horrified of what has happened the past 2 years when I’ve sought help or referrals. I wish I could give feedback to him for his own self-reflection improvement, but I also don’t want him to take it the wrong way and/or retaliate — I still need him to be my physical-health GP until I can transfer to the chronic-health clinic i’m on an indefinite waitlist for).

Re: TW: The 'real' me inside - I come with an SI warnng

Aww thank you @Shaz51 😍 that is lovely if you do! And guess what? my B’day is even in this second-half of the month, so it’s good timing for “upcoming” still lol. 😋 You're a sweetie 😍. My closest friend is visiting me this weekend, and so we’re going to do a “birthday dinner” even though it won’t be on my b’day that she’s here 🤗.

Would love to hear how things are going for you also, if you feel like sharing. You’re also on my list of people to check-in with when I have time to sink into reading ‘personal-threads’. Along with @creative_writer and @Blackcloud who I also know it has been ages since! I’m thinking of you too - and please just know it’s a crazy time for me, it’s nothing personal to you! I will get around to it eventually! So please do send me post here to share how you are if you’d like to. 😊

Whilst I use this as my space to share how I am, I also very much like it to be a space where any of my friends can also share how they are & their life updates too (esp if I am unable to back-read everyones threads when life gets too busy).
