Managing thoughts of suicide & self-harm

TW: The 'real' me inside - I come with an SI warning

TW: The 'real' me inside - I come with an SI warning

I figure it's better to introduce my challenges here than in a welcome. Hopefully only people willing to hear about SI will venture here.

TW: Suicidal Ideation

Content/trigger warning

I've had thoughts of suicide since I was 12. I don't remember much of life before this time. I'm aware there are times that I haven't though as much about suicide as others, but it's always been a "yes please, that would be my preference" even during the better times.

Why am I still alive then? For two reasons:

- the emotional pain it would cause to very close family.

- the physical pain of accessible methods, and the risks of non-fatal attempts causing lifelong physical impairments.

How do I keep myself going all these years?

By focusing on how I can benefit other people's lives instead. If I'm going to put up with life, it may as well be beneficial to someone.

And to help me keep life less monotonous I look for 'interesting' things - quirky people, exploring foreign countries/cultures, trying new foods (even if i dont think I'll like them). I've never been a status-quo person.

I explain to MH supports that when life is not distressing I can carry on without seriously considering suicide as a possible option. I can say "hello there thought, I hear you but I have chosen not to act on it, things are OK". But when something sets things off kilter, its a pretty quick track back to "why am I still doing this? Do I really still have the same commitment to living? And why, for whom? Why can't I be permitted to die if that's what I want for myself?"

With decades of this under my belt, I have the ability to sit with feelings of extreme distress. And the courage to text or call someone if I need to - and I'm getting a little better at doing this *before* it gets too extreme, but I still hesitate because I don't want to burden others and the frequency of it happening during those unbalanced times is too often. I've persuaded myself a few times recently to instead call a crisis line, but waiting 30-45min in a queue can sometimes exacerbate the thoughts, and sometimes the calls might help dissipate the feelings (if i called early enough) but other times I feel worse after because of course they're going to try to give some positive outlook for the future, but it grates on me because deep down I dont want a future at all. Its actually refreshing when someone acknowledges that not liking/wanting life is OK - and instead just tries to look at how to rebalance the scales to help me cope with continuing to live despite not wanting to.

I'm not sure how un/common my way of thinking is. So much about suicide prevention is focused on short-term distress tolerance or "fixable" problems (eg situations).

What do I need from the community here...?

To be heard? To know if anyone else out there has a long enduring preference to die even when they aren't feeling active distress? And if so, has anything helped them longterm to shift that thinking?

If anyone knows of any practitioners who *specialise* in chronic SI since a young age, I'd really love to know who (if there is a way someone is allowed to??). I've tried Google searching and using online psych directories and reading bio's. I've seen many psychologists and psychiatrists in the past who haven't been able to help me shift this (even if theyve helped in other ways).

I do wonder if my neural networks for positive emotional may not have fully developed or may have atrophied due to the young age and length of time. I genuinely do not know what being happy feels like, I do not have any memory that I recall as being "happy" or joy, and I suspect this makes recovery much harder.

Im ok at the moment. Im safe. But it is exhausting.

Apologies this post is quite a bit too long.

@Flutterbug9 @Cuddlebear

Another hour has passed and I forgot my wet laundry! This is very funny. I am off-forums for the night now, hang the washing to dry and to bed! Away with me! 🤣😂 My silly forgetful distracted squirrel brain.

Re: TW: The 'real' me inside - I come with an SI warning

I could request an appointment with my GP prior to May just for purpose of updating my meds. TBH, as much as it frustrates me that things are not up to date, I honestly can’t be bothered @AlwaysMyself
my appointment in May will be my usually 3 mth appointment. Check blood pressure, ask how things are going with my new psych (if GP remembers), new scripts.

my issue on Tuesday afternoon was, what started off as and should have stayed as general topic conversations, become “political based”. By now, I’ve done my own research online and have the fully informed. This has meant, somethings that were said on Tuesday afternoon, I know were incorrect.

earlier today I sent a message hto the person mentioning feeling confused following previous messages. Apology received and full explanation. All sorted. Atm we will go to a cafe in May for a catch up.

Re: TW: The 'real' me inside - I come with an SI warning

I see my psych one a fortnight, each second Thursday morning. Last week I took in couple items to do with model trains plus one Lego item. That led to few chats about both hobbies. This psych shows the emotions on her face and in her eyes. The excitement and interest when I open a box to do with model trains, it was like watching excited face of a young child. I like that side of the psych. Gives me the feeling they are “with me”.

Talking with current psych feels like talking with someone I’ve known for awhile. As I mentioned last week in the appointment, I always felt previous psych was in a position of authority. Immediate comment by my psych was about previous one not being the right match.

with getting things done, without any type of “due date”, I fall into a “ who care “ mode where things eventually get done. Atm I’ve got sheets still in laundry basket, waiting to be put away since they were washed mid last year. No one else sees the basket so why care.

having a specific cut off date definitely helps with getting things done. Energy and enthusiasm stay that way for awhile prior to slowly diminishing. Planning on taking advantage of the current focus and energy drive to get back into the garage tomorrow, focus will be to deal with one box or small section of things one day at a time.

most evenings my pattern is dinner, cross stitch whilst watching/listening to dvd followed by reading for last hr approx before going to bed.

im in the second smallest place ive lived in yet cant stay on top of things.

think I owe you some posts. I need to read back over things. Not tonight though

Your updates are always interesting @AlwaysMyself,

Thank you so much for letting us know how things are going. 🥰

I just wanted to double clarify that you are officially a part of the research trial? 🤩

How wonderful to have that validation from your therapist about neurodivergence!

And sooo amazing that you can feel more in your body. 🥹 I know it can be scary to feel more for the first time in a while, but I am so glad you are able to access more of your inner landscape and a sense of motivation. That must feel really relieving?

It sounds like it can feel more heightened too, but it's helpful to note that sensory sensitivity could be a contributing factor here too. I actually had a similar experience recently with medication... sometimes we do need a change. They are absolutely life-saving treatments, but it's very common for our bodies to adjust and change over time and need something different. I think this information about autism/adhd and learning about what your brain needs will be so life changing for you. I am so happy to read all of this and very excited to see where your discoveries lead. Please keep me updated. Proud of you!! 😍

Re: TW: The 'real' me inside - I come with an SI warning

Glad the meet-up with your friend was clarified and resolved/sorted @Flutterbug9 🙂 🙂 A happy outcome! (I hope)

I find it frustrating when I hear people talking about things as "fact" which are factually incorrect. Takes a lot of effort on my part to "bite my tongue" and not saying anything lol. But if it's in a conversation with me 1:1 or just 3 of us, I sometimes use "curious questions" to gently and subtly explore why they see those things as "fact" which I don't think are (but without actually saying i don't think they're fact! Unless they ask me or seem open to alternative perspectives).

Playing "curious questions" is emotionally exhausting though lol. Unless is about their MH - then that isn't so exhausting lol because it is interesting!

I love that you could discuss your hobbies & interests with your psych -- such a good way to establish rapport and share who you are with them! So good that they were also able to share the joy they felt in this too. 🙂

(It reminds me a little when my counsellor sees me express exuberant-annoyance or if I say that I have tried/tested out a way to release this feeling -- she gets very excited that I am feeling emotions beyond despair/depression. Of course she gets even more excited when she hears if something has been a motivating feeling because that it a positive thing.)

It sounds like the current psych relationship is heading in a really healthy direction and trust and honestly. 🙂 I am so pleased and glad to hear for your sake - you deserve to find someone like this! And I recall how hard it was initially to open up and try a new one after the last experience.

Your comments re: "due date" vs "who cares" mode -- Ditto for me. Hence why I was curious 😉 . My clean laundry stays on my drying-rack until I use things off it, and then when I need to hang a new load of wet clothes up then I take them off to put into a pile of clean clothes (I am trying to put them away now days, since there isn't many items at at time - but sometimes I still don't if I'm in a rush or not in a suitable mood to cope).

"one box" method is great!! 😄 I am wishing you luck and mood for the garage. 🙂
Sometimes I also do "just one item" because sometimes once I get started, then I can keep going, but it also gives me the permission to not keep going after the 1 item if i still feel it's "too much" currently. 1 item is better than 0 items!

PS - you never owe me posts/anything. 🙂
(I know you may be using it as a turn of phrase - which is 100% fine 🙂 But I just like to make sure to re-iterate the no-pressure!)

Re: TW: The 'real' me inside - I come with an SI warnng

@AuntGlow "interesting" is a word for it lol. "All over the place" might be 4 words for it? There is more I wish I could tell you, but can't.

Yes, should be officially part of the research trial - although pending my drug-test results from yesterday being sent to the team of course. I tried to call the GP this arvo for them to do it, but despite the online opening hours their phone/reception was already closed -- but thats OK because I can call/go tomorrow when I will be there for a different (fasting) blood test anyway.

Next date for the first "step" in the actual treatment/trial will be in 2 weeks (or so is planned, we know life changes things sometimes!), but first drug-dosage session won't be until the end of next month. ( @Till23 as i believe you are interested to hear also).

I still don't like the feeling of rapid-heartbeats and whole-body muscle spasms when I feel emotionally-excited/motivated by things. But I can tolerate it. I don't see it as "scary", more as "annoying" and physically-uncomfrotable lol.

My frustration tolerance has definitely also decreased though - i.e. I am feeling it more strongly too and more easily overwhelmed by it. It's a rare day that I do not end up crying (or feeling like crying) about things now - which is similar to how it was last year when I first joined the forum in the height of my stress with work.

The degree of emotional blunting with the medication... wow. I am glad that most people do not get that effect. I honestly don't know if it's been like that the whole time, or if it gradually got worse over the years. I saw something I had written within 1 year of starting the medication (but noting I had come off a different one and onto this one) which I had said "no change". But really interesting was that I had written that when I started the prior medication (3 years prior) I had noted "improved focus/concentration and decreased fatigue" despite no improvement in mood. I thought that was very interesting (and I have no recollection as it was nearly 20 years ago now) - given that I am finding focus/concentration much harder since coming off the medication (except when focusing on "special interest" areas) -- but I am also seeing that this type of lack of focus I have feels very much in line with what I hear/read for ADHD symptoms.

I'm currently pulling together information from various sources so I can give it to my GP for the DSP & NDIS applications, as a consolidated "history" and "symptoms". It is so time-consuming and I keep going off on interesting tangents. But I have found some really useful old Patient Intake forms I have submitted that have my accounts of treatment history (especially medication & treatment dates!). The problem with a 25 year history is that many files/letters between my treating doctors/psychologists will likely not have been retained because I did not transfer files when I moved interstate because I had given up on accessing medicare-funded supports so there was no need to. My new GPs just continued to write whatever scripts I was already on without wanting my past files. I guess we didn't know I might need them in the future, because back then there was no NDIS to need to prove "permanency" for -- it was all about "current symptoms" instead.

@Cuddlebear @Flutterbug9 @Jynx

Another distractible/forgetful brain laugh?

I sat down with a drink to forum-land about 3 hours ago... my drink is still unopened... I totally forgot about it. Too late now to open it to drink! Back in the fridge it will go lol. *this is so common for me* (and my dad was the same).

It always comes down to "how often is "normal" ?" haha

@AlwaysMyself, how’s things my friend? I send big apologies I haven’t been here much 🙏, always thought about as always.

on adhd have you heard a book called - the year I met my brain?, I am not sure if we have discussed this yet? If we have big apologies.

How was your time away?

Hows your week been and how’s the week ahead looking for you?

🌻

Re: TW: The 'real' me inside - I come with an SI warnng

Thanks for update @AlwaysMyself as I am interested in this trial and also how you are going in general. It is also interesting to hear you report about the emotional blunting on the antidepressant. I was on an antidepressant, sort of recently where I did not feel anything; I didn't feel particularly depressed but I didn't feel joy or have any motivation either. It was really difficult to do things, because I didn't feel particularly anxious either.

I am currently doing a trial, but it is a cancer related trial. I thought I might be ineligible due to my MH diagnoses, but I wasn't as I started the trial today.

Re: TW: The 'real' me inside - I come with an SI warnng

Hey @Chasingsunsets , no pressure on you ever 🤗. I havent been on heaps either, hence not posting on the highlights since the waves near sunset time i sent you.

Keep riding the waves, hopefully with a body/surf board to help keep you afloat when you're exhausted 🤗. Hope I/we can help be friends to be a part of that surfboard 😉 when you want/need some bouyancy support. And also for some fun, right? I guess my method of laughing at the stressful things can be kinda like surfing a wave yeah? 🤔

My friend mentioned the book to me at the start of the year. I have listened to the first half twice (once then, and again started recently - i still want to finish it). It was really hard for me emotionally to listen to it. And this second time i am trying to listen to it I am trying to note down all the reasons it makes me cry and feel overwhelmed (hint: because it resonates with life being such a struggle). But it means I cant lisen to it whilst driving, for example! I had the same thing (but less overwhelming because it was *after* my GP recommended the book because of suspecting autism - so then it felt more validating and less depairing) when listening to "the autistics guide to self-discovery". This time reading The Year I Met My Brain is a bit easier because I feel a little more "allowed to think it could be" since 2 doctors have suggested it to me in the past 6 weeks (its not "but isnt this normal? I've always been told i'm normal. Why am i relating to it so much if im not adhd? I must be over-reacting in how i feel!)

Time away was difficult. But there were a few good things.

Busy week and weeks ahead with lots of life admin, appointments and a few social things.

I am so glad i am not working currently, as i am already struggling day to day and week to week, but if i was trying to do everything i am currently doing whilst also working i don't think i could juggle it all without being utterly exhausted and despondant again. Trying to do as much as i can to get my DSP evidences collated for my GPs (hopefully a lot of this can also be reused but reformatted for NDIS - eg treatment history & symptoms/impacts on capacity). Realised today again that I *still* haven't listed my second car for sale yet, despite getting it fully ready for sale a month ago 😣 - but really it has the "latest deadline" but it would be good to sell it sooner whilst it has more rego left for the buyer (plus is still mostly clean exterior!) . I should prioritise a basic ad listing, since it will only take me 30min to do. I need to set an alarm for doing it to remind me! (But thats a normal thing for people do right?? Set alarms for doing important tasks because they have kept forgetting 🤣.)
