Managing thoughts of suicide & self-harm

TW: The 'real' me inside - I come with an SI warning

TW: The 'real' me inside - I come with an SI warning

I figure it's better to introduce my challenges here than in a welcome. Hopefully only people willing to hear about SI will venture here.

TW: Suicidal Ideation

Content/trigger warning

I've had thoughts of suicide since I was 12. I don't remember much of life before this time. I'm aware there are times that I haven't though as much about suicide as others, but it's always been a "yes please, that would be my preference" even during the better times.

Why am I still alive then? For two reasons:

- the emotional pain it would cause to very close family.

- the physical pain of accessible methods, and the risks of non-fatal attempts causing lifelong physical impairments.

How do I keep myself going all these years?

By focusing on how I can benefit other people's lives instead. If I'm going to put up with life, it may as well be beneficial to someone.

And to help me keep life less monotonous I look for 'interesting' things - quirky people, exploring foreign countries/cultures, trying new foods (even if i dont think I'll like them). I've never been a status-quo person.

I explain to MH supports that when life is not distressing I can carry on without seriously considering suicide as a possible option. I can say "hello there thought, I hear you but I have chosen not to act on it, things are OK". But when something sets things off kilter, its a pretty quick track back to "why am I still doing this? Do I really still have the same commitment to living? And why, for whom? Why can't I be permitted to die if that's what I want for myself?"

With decades of this under my belt, I have the ability to sit with feelings of extreme distress. And the courage to text or call someone if I need to - and I'm getting a little better at doing this *before* it gets too extreme, but I still hesitate because I don't want to burden others and the frequency of it happening during those unbalanced times is too often. I've persuaded myself a few times recently to instead call a crisis line, but waiting 30-45min in a queue can sometimes exacerbate the thoughts, and sometimes the calls might help dissipate the feelings (if i called early enough) but other times I feel worse after because of course they're going to try to give some positive outlook for the future, but it grates on me because deep down I dont want a future at all. Its actually refreshing when someone acknowledges that not liking/wanting life is OK - and instead just tries to look at how to rebalance the scales to help me cope with continuing to live despite not wanting to.

I'm not sure how un/common my way of thinking is. So much about suicide prevention is focused on short-term distress tolerance or "fixable" problems (eg situations).

What do I need from the community here...?

To be heard? To know if anyone else out there has a long enduring preference to die even when they aren't feeling active distress? And if so, has anything helped them longterm to shift that thinking?

If anyone knows of any practitioners who *specialise* in chronic SI since a young age, I'd really love to know who (if there is a way someone is allowed to??). I've tried Google searching and using online psych directories and reading bio's. I've seen many psychologists and psychiatrists in the past who haven't been able to help me shift this (even if theyve helped in other ways).

I do wonder if my neural networks for positive emotional may not have fully developed or may have atrophied due to the young age and length of time. I genuinely do not know what being happy feels like, I do not have any memory that I recall as being "happy" or joy, and I suspect this makes recovery much harder.

Im ok at the moment. Im safe. But it is exhausting.

Apologies this post is quite a bit too long.

Re: TW: The 'real' me inside - I come with an SI warning

Thanks @Jynx . The counsellor i'm seeing at the moment has training in existential therapy, so I am hoping that we can talk about that meaning/purpose side of things. I think she is starting to understand my context now, so hopefully she will be able to chime in a bit with delving into such things. Ive said a few times that for me currently the lack of meaningful or positive experience in life is what i think is most problematic for my thinking/feeling currently.

I stopped seeing professionals for therapy 11 years ago because there wasnt anything more the psychiatrist could offer to help. Id had 10 years of weekly psychology/psychiatry treatment by that time. Has taken a leap of hope to engage one again in the hope it may help and be worth the $$ every week. She was recommended as trauma-experienced.

@AlwaysMyself you're so welcome 😊

Another factor that might be worth reflecting on is your relationships with those therapists of the past. Everything I've read points to the fact that the modality or method is not as important as the therapeutic relationship itself. Unless we click with them - I don't just mean get along, I mean truly click, it can be so much harder to build trust, which is vital to having a successful working relationship where you can feel sufficiently challenged whilst being safely held in your vulnerability.

Hopefully this new person will provide you with that sense of trust and safety 🤞💜

hey @AlwaysMyself! just caught up and all the posts. i can see how after having 10 years of weekly therapy it would've been hard to jump back in, but i'm so glad you gave it another go - let us know how the existential therapy goes!

how'd the DnD game day go? i've always wanted to try playing DnD, it seems so fun but none of my friends are interested 😅 how'd it go chatting with your friends about your partner moving out?

hope this weeks been off to a nice start for you 💙

Re: TW: The 'real' me inside - I come with an SI warning

Thanks for checking in @rav3n .

DnD night was slow. It had some good creativity which i like, but I am still not finding it an enjoyable feeling. More just tiring and neutral when not irritated. I get a little irritated when people arrive late and then dont get started until much later. Often we haven't been starting to play until 2hrs after the agreed meeting time; it doesnt bother the others, only me.

Friends have been supportive with my partner moving out. Don't talk about it much other than to let them know.

I'm not doing well today. Came home from work early due to a mix of not having work to do, fatgiue and constant thoughts.

Re: TW: The 'real' me inside - I come with an SI warning

@AlwaysMyself 2hrs after the agreed time is quite late, i'd be frustrated too if it happened regularly. are they aware that their lateness irritates you? i have a friend who ALWAYS comes late to the point i give them a different arrival time to everyone else. if something start at 7:30pm, i say come by 6... they might get there by 8 🙃

the constant thoughts and fatigue would be exhausting! what would be helpful for you right now? do you prefer to rest up, maybe take a lil nap? or occupy yourself with other tasks?

Re: TW: The 'real' me inside - I come with an SI warning

I have told two of them it irritates me sometimes when we start late. 2 of the 4 are usually on-time. One is usually 30min late (which doesn't irritate me as much because I expect it) and we tell them to plan for 30min earlier. I am the only one in the group that isn't suspected ADHD - so their distractibility is understandable and not really something they can easily change. Sometimes I just try to find something else to do whilst they muddle around - but its not like I can go and read a book or have a nap.

I organised for a friend to pick me up on her way to a daily coffee meet (since I felt too tired and unmotivated to drive myself) to get myself out of the house. Then I had a lay down rest until then.

Still feeling pretty shitty, but fractionally better.

Yesterday I started creating a log of places I want to go see in USA - it will be a massive list, but by plotting it on a map I can then see what is or is not nearby each other or on a good driving route. It will take me a very long time to do this, as there are multiple states I would like to go to and I like to then look up what is in that state. So it would not be all one trip, or if it was it would be a multi-month trip. Depending how things go in Dec-March I would consider taking a few month holiday to just enjoy - the upside to my partner moving out is that I now don't feel as bad if I want to go away for a few months and I can expect him to look after the dogs instead whilst I do.

I'm going to "do a Shaz" and get a cup of (iced) coffee as comfort-drink treat to at least cheer my tastebuds if not my mood or life outlook.

Re: TW: The 'real' me inside - I come with an SI warning

@AuntGlow in answer to how I am (sorry, realising its now just past 10pm, so I know you will have finished work and thats fine I will also be heading to sleep now anyway).

Not good. Almost constant thoughts. Got a friend to come over Sunday night to keep me company until bedtime. Saw gp Monday (yesterday) and let them know how things have been - "ok, well please dont xxxx yourself; see you next time" (seriously not making this up, but also i mean yeah, I get it... what can he do?). Tried the warmline earlier today, but no one available at the time and I did not want to leave a message and without knowing what timeframe I'd get a callback (eg next day whilst at work). This evening was still constant thoughts so I decided I would try the suicide callback line (as I haven't used it before). Waited over 30min for the 20min chat slot. Wanted to talk/advice about whether (and if so, how/what) to tell a friend or two about the thoughts I am having, with regard to extra social support for times when I am having constant thoughts I cant get out of my mind. I explained my pros and cons of it to them, but they didnt have any comment on it other than to acknowledge that it is good to have a variety of supports. 😞

I'd like to have an in-person support (someone who knows me and not coming in blind esp because im not 'typical' in my thoughts/situation) that i can ask to come over or meet up when im having intensive constant thoughts. But I also dont want to be asking them frequently (which is also why having a few trusted people is an idea). And I dont want people to feel obligated to meet up if I ask, if they dont want to or are in the middle of something else.

I have ways of distracting myself and self-soothing. But part of the problem with these thoughts is that im not in distress, and if I have no desire to live I also have no desire to distract myself from future-planning/research. But I also know that planning/research is not healthy for my mind, so I try not to do it.

Ive spent most of the times of these thoughts and feelings alone by myself in the moment, not asking for support from friends. But I'm tired of bearing it alone, and it makes me feel even more lonely and motivated when I bear it alone. The upside of sharing it with a friend is that they have the opportunity to remind me they care and for me to not feel so alone in it in my face-to-face life.

The upside to feeling this way and crying is that now my body is ready for sleep, and I should hopefully sleep solidly until I have to get up for work tomorrow. ☁️ *silver lining*.

@rav3n @Jynx (just in case also wondering; i dont mind anyone responding to each other's responses).

@AlwaysMyself Hello beautiful. How was your day off? Was work ok today?

Im really impressed with your efforts to lose weight. Which meal replacements have you liked best so far? Are they filling? I have some low calorie Greek recipes that you might like? Happy to type them up if it helps.

What trial are you wanting to join?

It is hard isnt it leaving fur babies home. The good thing is they have one another’s company. Has your partner moved out for a trial?

Re: TW: The 'real' me inside - I come with an SI warning

Hey @Cuddlebear 😊

I hadn't tagged you in updates because I didnt want you to feel pressure to respond, especially while having the meds adjustments. Its so lovely to hear from you though 🥰😍 it has lifted me this afternoon to see your message.

I am hoping to get into a psychadelic assisted therapy trial (not sure if im allowed to mention which drug because of the forum guidelines, but one that is currently undergoing lots of trials for treatment-resistant Major Depression and PTSD treatment). Once I lose the weight I can continue going through the screening process to ensure i am eligible (including things like blood tests and an ECG). Its good to have a strong motivation to lose weight, and I hope I can keep my focus on that even though it would be at least 5 months until I can do the trial (and there is the possibility I wont be in the active group either, in which case I would have to wait until after the 6mth trial period to try to then find someone who can do the real treatment, which hopefully by then there might be in private practice).

The idea that it might be 1 year from now until I would get the real treatment is quite depressing and I have been struggling with that a little this week. But I am trying to stay focused on the shorter term and just getting into the trial for the possible treatment. (If I am in a control group, then I get a different drug which would be an interesting experience anyway 😅 as I have never used non-prescribed drugs).

I don't like the weight loss shakes generally. I struggle with the taste and and textures. Its been trial & error about which ones I find tolerable.

I found one type of vanilla ones and have tried adding it to my Iced coffee milk and also to some churros-flavoured Oat milk 😆 which improved it (although I tried this to other Vanilla ones and it wasnt lol).

I also bought some of the bars, and i find them much more palatable than the shakes. But I find i get hungrier later with them and dont find them as filling. Ive been trying to drink lots of water to help me feel full.

I dont like plain water (but doing my best), so ive also splurged on buying some of the flavoured Tonic Water (i dont like tonic water, but there is a raspberry one which is nice). 😊

I may occasionally have a little no-added-sugar chocolate or sugar free boiled lollies when I need a comfort food hit.

Yes, my partner has moved out for a while to see how he feels and whether he then wants to work on improving our relationship or split.

Am I currently in a relationship with him? Actually I dont know! 😅 he hasn't said lol. For all purposes it feels like a trial separation (ie not in a relationship), but its not actually been said, only that he would live separately. But in many ways I feel like I have been living like that for all of this year anyway, its just that he isnt in the house spare room anymore.

My mother is also away on holidays for another 2 weeks (we live in the same large house, since moving earlier this year). So I am also doing carer duties for her dog as well as my 2.

Work was productive today. 😊 I think ive helped discover a cause of an issue to resolve, which some people were in disagreement over.

How has your afternoon/evening been?

Re: TW: The 'real' me inside - I come with an SI warning

Good evening @AlwaysMyself 💖

This must be so overwhelming for you... how are you feeling safety-wise tonight? And do you have a safety plan that feels good for you at the moment? (Apologies if this has been asked before! It's helpful for me to know) ☺️

Goodness, that GP's bedside manner doesn't sound overly supportive or warm. I would have left feeling vulnerable and deflated... was it helpful having your friend with you on Sunday night?

Gah, I am sorry the helpline was unable to give you more... would you like to talk out some ways you could approach your friends? Here to help you brainstorm.

I hear you, someone in-person who can help you to co-regulate would be wonderful in those more intense moments. I know we discussed a support worker on @NightFury's thread. Maybe this could be helpful to organise?

Are you able to connect to one or two things that help you to feel hopeful or joyful?

I understand, and holding this alone would be so much for your mind and body; especially when you are needing support and care. I think increasing your support network is a great idea. Know that there are people here who genuinely care about you so much.

How was work today?

Also, I have been through a similar thing with my partner recently, so please tag me if you need to talk. 🫶
