Managing thoughts of suicide & self-harm

TW: The 'real' me inside - I come with an SI warning

TW: The 'real' me inside - I come with an SI warning

I figure it's better to introduce my challenges here than in a welcome. Hopefully only people willing to hear about SI will venture here.

TW: Suicidal Ideation

Content/trigger warning

I've had thoughts of suicide since I was 12. I don't remember much of life before this time. I'm aware there are times that I haven't though as much about suicide as others, but it's always been a "yes please, that would be my preference" even during the better times.

Why am I still alive then? For two reasons:

- the emotional pain it would cause to very close family.

- the physical pain of accessible methods, and the risks of non-fatal attempts causing lifelong physical impairments.

How do I keep myself going all these years?

By focusing on how I can benefit other people's lives instead. If I'm going to put up with life, it may as well be beneficial to someone.

And to help me keep life less monotonous I look for 'interesting' things - quirky people, exploring foreign countries/cultures, trying new foods (even if i dont think I'll like them). I've never been a status-quo person.

I explain to MH supports that when life is not distressing I can carry on without seriously considering suicide as a possible option. I can say "hello there thought, I hear you but I have chosen not to act on it, things are OK". But when something sets things off kilter, its a pretty quick track back to "why am I still doing this? Do I really still have the same commitment to living? And why, for whom? Why can't I be permitted to die if that's what I want for myself?"

With decades of this under my belt, I have the ability to sit with feelings of extreme distress. And the courage to text or call someone if I need to - and I'm getting a little better at doing this *before* it gets too extreme, but I still hesitate because I don't want to burden others and the frequency of it happening during those unbalanced times is too often. I've persuaded myself a few times recently to instead call a crisis line, but waiting 30-45min in a queue can sometimes exacerbate the thoughts, and sometimes the calls might help dissipate the feelings (if i called early enough) but other times I feel worse after because of course they're going to try to give some positive outlook for the future, but it grates on me because deep down I dont want a future at all. Its actually refreshing when someone acknowledges that not liking/wanting life is OK - and instead just tries to look at how to rebalance the scales to help me cope with continuing to live despite not wanting to.

I'm not sure how un/common my way of thinking is. So much about suicide prevention is focused on short-term distress tolerance or "fixable" problems (eg situations).

What do I need from the community here...?

To be heard? To know if anyone else out there has a long enduring preference to die even when they aren't feeling active distress? And if so, has anything helped them longterm to shift that thinking?

If anyone knows of any practitioners who *specialise* in chronic SI since a young age, I'd really love to know who (if there is a way someone is allowed to??). I've tried Google searching and using online psych directories and reading bio's. I've seen many psychologists and psychiatrists in the past who haven't been able to help me shift this (even if theyve helped in other ways).

I do wonder if my neural networks for positive emotional may not have fully developed or may have atrophied due to the young age and length of time. I genuinely do not know what being happy feels like, I do not have any memory that I recall as being "happy" or joy, and I suspect this makes recovery much harder.

Im ok at the moment. Im safe. But it is exhausting.

Apologies this post is quite a bit too long.

Re: TW: The 'real' me inside - I come with an SI warning

I get the memory side of things @AlwaysMyself

I have multiple alarms set within my phone for everything. Works really well up to 8pm when I have my phone set for sleep mode, one thing I do to help with sound sensitive. Downside of this phone is bedtime alarm becomes silent. Only workaround for me is to set timer every night. One additional thing which, with my current stress levels, results in tension headaches.

ADHD plus CPTSD question about feeling restless or fidgety. Recent nightmares and memories of preteen being forced to stop it. unplanned GP appointment yesterday, mention was made I’m subconsciously doing it now. Atm on meds to help with inflammation and irritation plus mixture to help with the cough. Needing info from recent tests as next step

Being tagged. I don’t have the time or energy to be logging into the forums all the time. I try to log in asap after getting email notification I’ve been tagged in a post. At times can be a week between checking my emails.

i like the way my psychologist talks about autistic traits/habits/must do (whatever people are comfortable referring them as). Psychologists has said a few times, common comment from adults diagnoses/assessed as autistic is “doesn’t everyone do things that way”.

ive been misdiagnosed or has been miss diagnosis (plus other medical related things) large number of times, starting from when I was early teenager that I’ve lost a lot of confidence and trust in the entire wide medical field. Having gone through 2 cancer scares, due to appropriate scans not being done, in my late 30’s/early 40’s being 2 of them

’family’ (cousins) cause some challenges for me. One in particular fully believes, due to her uni qualifications as a teacher, she has more knowledge than others. On flip side, one of her nieces tells everyone she has self assessed autism, crohns and POTS. Fully expect if i said im self assessed autism ill be treated with same brush ie disbelieved. Only relatives who ever fully understood me died when I was 12 and 13 yo (ie 2 different people).

my health and my cats are my top priorities, being on the forum is low priority.

Re: TW: The 'real' me inside - I come with an SI warning

@AlwaysMyself I have been known to do similar with laughing. I’ve been told is coping mechanism for me for dealing with high stressfully conversations. Laughter being an attempt to lighten situation and/or distract away from stressful topic

my sensitivites

Sound - both volume and pitch. Hence I now have 2 or noise reduction ear plugs, different levels of noise reduction and have noise cancelling headphones. (Due to unrelated issue, I have inability to hear things that are very quiet)

background sounds and conversations cause me to get distracted which leads to feeling of overwhelming and dysregulated. (Hence issues Thursday evening, group meals, high pitch and tone cause issues at Trivia and on Thursday evenings)

Lights - unless I’m exhausted, I need total dark to sleep. Inability to tolerate bright lights

visual - sight of overcrowded areas

touch/tactile - feelings of discomfort and anxiety when hugged by anyone unless I have already established strong bond with. My entire body stiffens and heart rate increases when anyone hugs me. Same as when people get physical too close ie in my personal space.

Gardening and working on some projects very emotionally challenging due to high level of discomfort with soil, glue etc on my hands. I have made my finger tips bl33d when trying to remove stubborn glues that haven’t fully responded to water and soap

high tolerance to most pain, quite possibly as have “gotten used to” chronic pain

weight from blankets I find calming (linkage to CPTSD also) and would use all year round if could tolerate the heat in warmer months

intolerate of extreme temperatures for both highs and low

Balance and movement - fear of falling, being upside down, not able to have solid ground below my feet (am aware of these since being in primary school)

I also struggling with

- time management (multiple alarms set leading up to ‘event’ to ensure I’m not late, judging how much time needed to complete things)

- social challenges including reading and understanding social cues

- requirement for routine to prevent feelings of being overwhelmed. This including not changing location of things. Recently tried to reorganise things in my room. Emotionally painful lesson, felt dysregulated for couple days until put things back where they had been.

aspects of Schema Therapy last year helping reduce impact of perceived rejection in certain circumstances. Only small benefit from approx 2yrs with previous psychologist

I know you will respond when you can (no mention of any possible timeline helps stop the perceived rejection dark thoughts). Know that might be this week, next week or neither. My concerns were and still are, impact on stress levels for you due to impact of possible backlog of tagged notifications. Shall keep my concerns to myself from now on in attempt to prevent anymore possible misunderstandings

Hello @Flutterbug9 🫶

I can hear you have self-assessed for autism - I think it's really courageous to explore a deeper understanding of your mind, (albeit, an overwhelming process at times too!)

It's interesting what you have shared about people relating to things differently, and their individual relationship to overwhelm too.

Our brains are indeed wired differently, which is why understanding our own uniqueness can be so empowering.

I know it can feel awfully isolating too, so I don't want to discredit how vulnerable that can feel either.

Will you continue the discussion around autism with your psychologist?

Re: TW: The 'real' me inside - I come with an SI warning

@Flutterbug9 never any pressure from my to check emails for tags, or even to check the posts when tagged 🥰. I think its very healthy that you are limiting how often yoh check the emails to be only when you want to. 😊. Forums should be a thing for you that is supporting and/or enjoyable or meaningful -- definitely not a "chore" or "must do".

Be reassured I will never be upset by a lack of reply or long delay. Its all part of the forum ethos to accept and allow that. 😊 (but i do like chatting with you, so please dont think i say this out of a negative motive. Only from good healthy ones of respect).

Re: TW: The 'real' me inside - I come with an SI warning

I love how much insight into your autistic traits you have @Flutterbug9 😊. Of course not completely the same, but i definitely share a good portion of them with you.

Autism + CPTSD is definitely a harder thing to figure out whats due to what, or so i have been told by psychologists. I think mine "had it lucky" since i dont have PTSD despite trauma (i actually believe my autistic traits and adhd memory have been very protective factors for me in this, as i was able to treat my early ptsd quite easily with cbt). I am grateful I don't have that complication, and feel a lot of empathy and compassion for yourself and others with C/PTSD (and anxiety or bipolar, etc -- anything "extra complicated").

Im curious how the noise cancelling headphones are going, and as you noted you that you can have unrelated difficulties hearing things at low volumes whether these headphone make it harder to hear with respect to that also? Or whether the headphones are designed to pick up the 1:1 voice interaction to boost it to help still hear / target that sound?

Technology is pretty amazing with what i can help isolate and boost now 😊. I have a number of friends with hearing aids/devices (not necessasily headphones in their cases) and its pretty impressive!

Had a long day out today with 3 appointments, but they all went well. So i figured i'd check in my emails from this morning / last night to wind-down on the couch. 😊. But i havent checked the ones from a few days ago yet; they can wait until i'm more awake. I feel like having a nap from the tiring outing, but i am concerned that if i do then i won't be able to sleep later tonight. ...maybe i can get away with a 30min nap though to boost me. I have some housework i wanted to do this evening.

Tomorrow is another busy day (leaving home 730am, back home about 3pm, then out again at either 4pm or 6pm until late-night social thing). Its going to be so noisy tomorrow night 😏 but i need to be able to hear people to talk to them 😏. Ahh i wish they all did Auslan 😆.

Saturday i'll also be out most of the day, i think 730am until 5pm or 6pm.

Looking forward to Sunday when maybe i dont need to leave the house!! 😆 sleep in sounds soo good for a Sunday plan!

Hello @AlwaysMyself, how’s your day been? Just wanted to drop in to say there is a reason it’s taken a bit of time to get back to you, just been in great thought about my reply🤩, I hope that I’ll be able to get to posting a msg to ya tonight or tomorrow, sending hugs and definitely have not forgotten just working on a response my friend. 💜

Sharing this pic below makes me want another puppy 😂🥰🐶

Re: TW: The 'real' me inside - I come with an SI warning

Main topic of discussions with my psychologist recently has been autism and the different way our brains work @AuntGlow

Earlier this year I told my psychologist of my goals which we will go back to at some stage. I’m starting to see where autism connects into some of the struggles behind my goals and my not understand some of my decisions/reactions last year.

i will definitely be continuing the discussions with the psychologist and hoping to discuss with my new psychiatrist, initial appointment in June. My psychologist has already offered to write letter to my psychiatrist. First time in my 60+ yrs of life I can remember this much support and understanding

Forums have become feeling like too hard work and a place I don’t fit into @AlwaysMyself

I want and need the connection at times, majority of the time I crave silence and solitude. At times need to be able to verbalise things, apart from psych appointment, forums and my supports are unable to help due to their structures. Recently spent hours at local MH centre, struggling for words at times, being able to talk out aloud and get out of my system huge amount of emotional pain.

logged on to ask the following and found 3 new posts 🙂.
anxiety starting and need to start to psych myself up to going out. I thoroughly enjoy the evening once the game starts, it’s the build up etc that causes distress. Makes sense for other things.

if you are ok to share - any thoughts, pleasant feedback on different colour text I used. Using different colour text is something I’ve done in certain situations for decades eg different Colour text in different locations within spreadsheets.

i haven’t used the noise cancelling headsets as yet. Bought for use in hospital and other settings where I want/need to listen to music/songs to help with regulation and concentration without everyone else hearing. Small in ear devices that come with mp4 players, mobile phones etc I can’t tolerate in my ears.

unrelated hearing challenges is I’m partly deaf, one ear worse than the other. Diagnosed by audiologist decades ago. Trials by audiologist with hearing aids helped with some things whilst making other things worse eg noise from tinnitus became painfully amplified. From things I remember my mum saying, I suspect I’ve had hearing issues majority of my life

I can understand you looking forward to Sunday and having entire day to do only what your body says to do.

id be interested to hear about the social event if is possible plus you are ok to share. Hoping it’s more enjoyment/less draining than the Shakespeare night (sorry if I’ve got the title wrong) few months ago.

i also enjoy our chats. The general topics, boring daily things etc. given time and taking things slow, we might get back to them. Today’s chats have been ones I’ve found comfortable for awhile. 😊

starting to rain plus is dark outside. Quick change of clothes and I need to get going.

Re: TW: The 'real' me inside - I come with an SI warning

@AlwaysMyself it took long enough but... I'm glad you do finally have a support team around you who by and large seem to understand you, your needs, and can understand what is going on for you with things like 'inappropriate laughter' or speaking up about certain things when they're super important.

@AlwaysMyself wrote:

It begs the question.. where is the definition of a “personality disorder” vs a “personality due to neurotype, which is normal within that neurotype”.

I think about this a lot. I think about how, depending on the person doing the diagnosing, and the day (i.e. what is going on for me internally), it's possible I could have been diagnosed with all sorts of things. What is neurobiology, what is trauma, what is intergenerational trauma that lives within neurobiology? I'm listening to an audiobook off and on atm called My Grandmother's Hands by Resmaa Menakem and he goes into it a bit. Definitely recommend!

One of my fav quotes from it so far:

Re: TW: The 'real' me inside - I come with an SI warning

Hey @AlwaysMyself, how have you been? Sorry it’s taken so long. Honestly massive apologies.

How were your appts today? Did you manage a nap? I can’t do naps, I always sleep for ages 🤭 lol. It sounds like you have a couple of big busy days ahead, hope they are good days for you! I love your Sunday plan 😂😂- recharge those batteries 😉🔋

Like @Jynx I also have wondered about you being a spokes person or something to advocate for change within the mental health sector. You would be amazing at it. Interesting to read you have recently done some, re awaiting the call back. I see though, at the moment you have a lot on re appts etc that timing isn’t right but I definitely could see you somewhere in that field or writing some really awesome letters. After all lived experience is one of the most important stories, real evidence nobody can question (they can always try). Your passion for change of these system failures really shines though honestly.

Sorry to hear you are still having trouble with your blood pressure, is this a new thing? It was interesting to read you don’t really want to go to hospital even for physical things. I wonder why your blood pressure goes so high? Have the doctors got a plan for ya? You said possible risk of stroke, sounds worrying I’m with you there 🫶. I hope you’re ok re your recent hospital trip for blood pressure 🙏

So April is the start of the trial? Since you have received the diagnosis of ADHD and autism I am wondering how you are feeling about the trial now? Does it change anything for you? (Pre start, just your thoughts/feelings) I am really interested with how it all goes. Fingers crossed for some good outcomes right, I wish you nothing but the best of course 😊. I have an idea what hormone it will work on - actually not sure if I can mention? I’ll mention as seems harmless but mods can delete if not allowed- sorry I forgot the rules with this stuff. So meant to work on serotonin mostly? Then loose effects on dopamine and glutamate? Gah I love talking psych lol - interests me so much. The brain is super interesting. (There’s our word again 😉)

I assume there are plans of what happens if you don’t respond well to the trial? Re hospital if really required etc. Also from what I was reading it ?could possibly increase blood pressure the trial but I am sure you and your team are all over all that ☺️

I am totally with you on your thoughts about grief and the lack of providers that believed you. There is some on my list- I just want to reclaim my power but I’ll only ever get one shot at it I think. So I am also at the point is it worth putting my effort into and what do I want the outcome to be? Will it change anything. How else can I reclaim my power back? I have been wondering this for such a long time. The feeling of being essentially failed hits incredibly deep, they are in a position of power and trust (at the start we think “they must know”) can I just say F*** we couldn’t have been more wrong. There is some small victory of finally a diagnosis but to know we went our whole lives being “missed” “overlooked” it really hits deep. It hits incredibly deep for me and I haven’t really shared it all that much. What makes me humble- which I think I have mentioned before but a family member close to me has definitely 💯 hyperactive adhd and ocd. Untreated, due to her age she is essentially out of the bracket to even try anything. She’s has lived her whole life like this and managed as best she can but life definitely has not been easy for her and for this I am grateful even though incredibly late, incredibly late not to save my soul crushed a million times over from mistreatment and how hard life has been, there is always someone worse and I am thankful for the opportunity that my family member didn’t get. 🫶⭐️🫶☀️💛

where are you at with your health professionals. I am glad the one you had decided to help you with your DSP stuff what a relief.

I am so glad you have a good team around you now and the mental health GP sounds great 🤩, I wish we had more of them honestly. How’s all the DSP stuff going, just waiting to hear or still submitting documents?

I was wondering, if you could wish one thing re support in crisis what is it do you think would be the best for you? Re type of care/service💛. Another heavy question, (again only if you want to answer)- what do you think it could take for you to want to be alive? - if you had one dream/wish relating to being alive, what would it be? Sending warmth and care to you. 🙏

It’s been lovely to catchup, thanks so much for all the tags and updates 🫶, I’ll wrap up here, please let me know if I have missed anything you wanted me to respond to as-well💜☀️. Take care, my friend.
