Managing thoughts of suicide & self-harm

TW: The 'real' me inside - I come with an SI warning

TW: The 'real' me inside - I come with an SI warning

I figure it's better to introduce my challenges here than in a welcome. Hopefully only people willing to hear about SI will venture here.

TW: Suicidal Ideation

Content/trigger warning

I've had thoughts of suicide since I was 12. I don't remember much of life before this time. I'm aware there are times that I haven't though as much about suicide as others, but it's always been a "yes please, that would be my preference" even during the better times.

Why am I still alive then? For two reasons:

- the emotional pain it would cause to very close family.

- the physical pain of accessible methods, and the risks of non-fatal attempts causing lifelong physical impairments.

How do I keep myself going all these years?

By focusing on how I can benefit other people's lives instead. If I'm going to put up with life, it may as well be beneficial to someone.

And to help me keep life less monotonous I look for 'interesting' things - quirky people, exploring foreign countries/cultures, trying new foods (even if i dont think I'll like them). I've never been a status-quo person.

I explain to MH supports that when life is not distressing I can carry on without seriously considering suicide as a possible option. I can say "hello there thought, I hear you but I have chosen not to act on it, things are OK". But when something sets things off kilter, its a pretty quick track back to "why am I still doing this? Do I really still have the same commitment to living? And why, for whom? Why can't I be permitted to die if that's what I want for myself?"

With decades of this under my belt, I have the ability to sit with feelings of extreme distress. And the courage to text or call someone if I need to - and I'm getting a little better at doing this *before* it gets too extreme, but I still hesitate because I don't want to burden others and the frequency of it happening during those unbalanced times is too often. I've persuaded myself a few times recently to instead call a crisis line, but waiting 30-45min in a queue can sometimes exacerbate the thoughts, and sometimes the calls might help dissipate the feelings (if i called early enough) but other times I feel worse after because of course they're going to try to give some positive outlook for the future, but it grates on me because deep down I dont want a future at all. Its actually refreshing when someone acknowledges that not liking/wanting life is OK - and instead just tries to look at how to rebalance the scales to help me cope with continuing to live despite not wanting to.

I'm not sure how un/common my way of thinking is. So much about suicide prevention is focused on short-term distress tolerance or "fixable" problems (eg situations).

What do I need from the community here...?

To be heard? To know if anyone else out there has a long enduring preference to die even when they aren't feeling active distress? And if so, has anything helped them longterm to shift that thinking?

If anyone knows of any practitioners who *specialise* in chronic SI since a young age, I'd really love to know who (if there is a way someone is allowed to??). I've tried Google searching and using online psych directories and reading bio's. I've seen many psychologists and psychiatrists in the past who haven't been able to help me shift this (even if theyve helped in other ways).

I do wonder if my neural networks for positive emotional may not have fully developed or may have atrophied due to the young age and length of time. I genuinely do not know what being happy feels like, I do not have any memory that I recall as being "happy" or joy, and I suspect this makes recovery much harder.

Im ok at the moment. Im safe. But it is exhausting.

Apologies this post is quite a bit too long.

Absolutely. For me, novel treatments can be a bandaid. They help you feel better for a bit, but if the underlying issue is not dealth with, it ultimately takes over @AlwaysMyself

However, this is not to say that you shouldn't try those things. Sometimes those little novel treatments and distractions give our bodies a bit of breathing space. We all know it's not the ultimate answer, but any shift is better than no shift - esp when things are tough

The hope with PAT is it can awaken dormant neural pathways @tyme . We've tried everything other than ECT or novel treatments (or endless medication cocktail experiments - pdocs and gps have said what ive been in past 15yrs is "best option" for me despite it obviously doing nothing (given i have had no decline in mental state whilst tapering. Tomorrow i will reduce to 1/8th of my dosage, which is my last planned drop before being off the medication completely).

Since nothing else has worked to shift the anhedonia, novel treatments are the last resort. End of the line, as it were. But i guess from other peoples perspective it also "buys time" since Oct last year until the end of the trial.

And form experience, what I found is that sometime might not work, but later down the track it does... it can be to do with whether our bodies/minds are ready @AlwaysMyself

Sometimes, yes @tyme . But how long does one have to wait for that?

I've been this way for 25 years. Initially 9 years with active weekly treatment. Then 15 years of self-management using those skills.

Sometimes psychotherapy and medication doesnt work. And i've already stayed around for too many years for the sake of others just in case something i try (changes to work, life, etc) helps. There is no sense in staying longer without a quality of life.

Cool @Cuddlebear ! That's pretty awesome. It's like what I'm doing now. I'm at the end of mine though. We're at the final write up.

For me, it was about 15 years @AlwaysMyself

@AlwaysMyself I hope you’ve got car back. I didn’t realise you were selling it. Are you planning on buying another car?

Last week it helped a little while I was doing job tasks. It’s not a huge number of hours though. I’m finding it hard to get any joy none whatsoever which I know we’ve discussed before.

Are you home resting?

Re: TW: The 'real' me inside - I come with an SI warning

Glad to hear it was a little better whilst doing the job hours @Cuddlebear . 😊 some distraction is better than none, i hope. Even if its not a "joy". I aim for "neutral" - that is, "not a negative experience" myself.

I have a second car, because when my ex was with me and he learned to drive then we found we needed a second car for when he wanted to take the dogs out when i was elsewhere, for example. So my dad gave me his old car, hence i had 2 cars. When my ex left he decided to buy his sisters car (since she moved overseas without a plan to move back and isnt using it) instead of buy my car (which is a little more expensive, because her old car is very very old). I am keeping the larger car that my dad gave me, because i wanted it for camping and being able to take the dogs along with gear.

Just asked for an update on the car. Wont be done until 7pm. *sigh*

At my grandmas. Guess i will need to have dinner here too.

Re: TW: The 'real' me inside - I come with an SI warning

@AlwaysMyself Yes I’d say it’s a neutral but that is ok much better than negative. It’s a step up as I see it.

Ah ok good idea then to sell the smaller one so you’ve got room for camping and dogs. Also much cheaper to run 1 car too.

What a shame they’re still not finished. 7pm sigh is a bit late for a Sunday but at least it will be clean and ready for sale. They usually do such a good job in the cleaning too.

Yes it looks like dinner at your grandmas. What’s on the menu?

Re: TW: The 'real' me inside - I come with an SI warning

Yeah, i dropped the car to them at 8am @Cuddlebear . Perhaps they also had a few things to do during the day also and thus took breaks. Its a persons small business from their home. I havent had a car cleaned by other before. I hope they do a good job. If they do, then when i want the other car done i could get them too also.

But part of me is concerned that they are not charging enough for how many hours of work. But i guess if there is mostly water, a bit of soap and vacuuming etc there is not much "expense" for them on top of their time, so then it is not a bad hourly rate i guess!

We will all have different things to eat for dinner. My mum cooks something fresh for my grandma, which tonight she wants sausages. But neither my mum or I want to eat sausages, so we will have something else from the freezer. My mum used to cook a range of meals for my grandma and put them in her freezer to heat up, but my grandma does not bother to so they have sat there for a very long time. So now we are eating them 🤣.
