Here Are Ten Things I Wish Would Change

Here Are Ten Things I Wish Would Change

1. I would like it to be mandatory for mental health patients to be interviewed in the presence of a carer, at least for part of the interview.

2. There needs to be a dramatic increase in psychiatrists. Every regional centre should have a psychiatrist.

3. Every public hospital should have at least one resident psychiatrist.

4. There needs to be a special benefit available to carers who are caring for several people with a disability or who need to give up work to be able to do so, and it needs to be about 10 times as much as the current carer's payment.

5. Police need to do an inservice annualy on Mental Health issues as they affect families. Particular emphasis needs to go on how to assist families in which someone has gone missing.

6. Psychiatrists also need to do inservices on how Mental Illness affects the whole family and in particular, where most or all of the family has MI issues.

7. CAT teams should be replaced with a regional service in which every major town has a psychiatrist, a psychiatric nurse and a social worker who visit you in the home during a crisis.

8. The number of beds for psychiatric patients in public hospitals needs to dramatically increase. A large weight of emphasis for decision making about hospitalization should be with the carer.

9. The government needs to employ service personnel who will pose as visitors but are secretly observing the staff on psyche wards to check on their bedside manner and level of care.

10. There need to be separate areas in psychiatric wards for patients who are not violent, loud or angry and need to be in a place of quietude.

Re: with the carer - Here Are Ten Things I Wish Would Change

Hi @perseverer

Thank you for your additional comment and I am glad that you reieved a postitive result from your complaint.

I probably should explain the motivation for my comments, as a new member I have been trying to share something as a new thread as I am feeling a little lost at the moment but don't know where to start, see how I go in the next couple of days. (Apologies to all if this should be as a separate thread).

I personally will never go to an ED or public hospital again if it is at all within my control.
(Really bad experience a number of years ago where my wife and I were made to feel like fools and that made a difficult situation situation much worse). I would have filed a complaint at the time but was told that they could make things difficult for us with regards to our children if I did (basically blackmailed me to shut-up). We now have private health cover giving us more choice with care providers.

That said my little rant was directed at ALL mental heath 'professionals' practically private.
My wife (18 years) was diagnosed with BPD and major depression aprox 5 years ago (anxiety and depression 10 years ago after the birth of our first child)

Until recently (with a new psychiatrist) I have felt inferior and completely insignificant when trying to engage with her psychiatrist/ psychologist (with her consent). At one point she was told to leave me (by psychiatrist) as I was aparently too controlling (if trying to obtain adequate support and create a safe environment makes me controlling then guilty as charged). I just don't understand how health care professionals think they can possibly provide the best treatment and support without feedback from primary carers, family and those closest, practically when the illness sometimes gives the person suffering an alternate version of reality. Never have I intentionally spoken down about my wife, only identified what the illness is doing to her at a point in time out of love and concern.

While with the new psychiatrist I have felt a lot more valued, (in fact been thankful for the support provided) in the last week there have been decisions made about potential new treatment without my inclusion that I am practically uncomfortable with, and I am having a hard time seeing any credibility with so called 'informed concent' when someone is in distress and not concerned with risk, especially as the perceived risk potentially affects our whole family. (Feeling anxious and alone at the moment).
More about this in the new thread if and when I get to it.

Hi @Shaz51

@Former-Member created a new thread lack of carer communication-how to deal with it , thanks Darcy, that was a big help.

I also created a new thread ECT for wife with BPD and depression- feeling anxious here and in the lived experience forum.

I have to say I am so happy to have found this space and be here on sane as people have been so helpful 🙂 it has also helped me to remember that I am not alone. (I already know this but easy to forget somtimes).

Re: Here Are Ten Things I Wish Would Change

Hi @perseverer

I got a response from the health service today. They said
1. They have had discussion as to how best to tackle staff education to carers and have had a meeting with a senior carer liaison officer to get things started.
2. They said that I should raise the issue of minimum contact direct with the case worker.
3. They are reviewing the role of the case worker so there is better hospital-community transition.
4. That better communication of external support should be communicated to carers.
5. That having an e-health record would provide access to discharge summaries.
6. That building services had been consulted in relation to a lack of privacy for one of the community consulting rooms.
7. That the clinical director would assist in helping me complete Mr Darcy's mental health chart.

The communication training was something I asked for as an outcome of the meeting. Will mull over their response.

Hey @perseverer
Met with clinical director again today. They are definitely looking at doing some training in relation to carer communication. I will follow up in 6 weeks to check on progress.

Re: Here Are Ten Things I Wish Would Change

Another thing I want changed that off-label prescribing only be allowed with documented patient consent and that involuntary patients should not be compelled to take them.

Re: Here Are Ten Things I Wish Would Change

I will second that @Former-Member

Had a problem here this afternoon whit the pharmacist dispensing a different brand.
The biggest problem I have in this instance is that the new brand came in a bottle not a box with meds in a perforated tray. Makes it a bit hard when I have to lock that particular med away and leave out enough for my darling to take it herself as required. (Usually I leave out 2-3 days worth at a time so she can be independent) but it is certainly not safe to leave it all out. Asked if it could be changed and was told that's all we have. It would have been nice to be asked if it was ok to provide the substitute before being dispensed.

Re: Here Are Ten Things I Wish Would Change

@Former-Member @perseverer

It is not a problem for us now but some time ago I went into the pharmacy and requested a note on our file (my Darlings file) that I be contacted if she had any scripts filled that may create a problem for us. There is apparently no such facility even with a medical power of attorney and they were not interested in helping me. I think somthing like this should be possible.

In our case my darling would only ever go to the same pharmacy so even at a local level it would have releived a lot of anxety at that time. What are your thoughts?
