Managing thoughts of suicide & self-harm

TW: The 'real' me inside - I come with an SI warning

TW: The 'real' me inside - I come with an SI warning

I figure it's better to introduce my challenges here than in a welcome. Hopefully only people willing to hear about SI will venture here.

TW: Suicidal Ideation

Content/trigger warning

I've had thoughts of suicide since I was 12. I don't remember much of life before this time. I'm aware there are times that I haven't though as much about suicide as others, but it's always been a "yes please, that would be my preference" even during the better times.

Why am I still alive then? For two reasons:

- the emotional pain it would cause to very close family.

- the physical pain of accessible methods, and the risks of non-fatal attempts causing lifelong physical impairments.

How do I keep myself going all these years?

By focusing on how I can benefit other people's lives instead. If I'm going to put up with life, it may as well be beneficial to someone.

And to help me keep life less monotonous I look for 'interesting' things - quirky people, exploring foreign countries/cultures, trying new foods (even if i dont think I'll like them). I've never been a status-quo person.

I explain to MH supports that when life is not distressing I can carry on without seriously considering suicide as a possible option. I can say "hello there thought, I hear you but I have chosen not to act on it, things are OK". But when something sets things off kilter, its a pretty quick track back to "why am I still doing this? Do I really still have the same commitment to living? And why, for whom? Why can't I be permitted to die if that's what I want for myself?"

With decades of this under my belt, I have the ability to sit with feelings of extreme distress. And the courage to text or call someone if I need to - and I'm getting a little better at doing this *before* it gets too extreme, but I still hesitate because I don't want to burden others and the frequency of it happening during those unbalanced times is too often. I've persuaded myself a few times recently to instead call a crisis line, but waiting 30-45min in a queue can sometimes exacerbate the thoughts, and sometimes the calls might help dissipate the feelings (if i called early enough) but other times I feel worse after because of course they're going to try to give some positive outlook for the future, but it grates on me because deep down I dont want a future at all. Its actually refreshing when someone acknowledges that not liking/wanting life is OK - and instead just tries to look at how to rebalance the scales to help me cope with continuing to live despite not wanting to.

I'm not sure how un/common my way of thinking is. So much about suicide prevention is focused on short-term distress tolerance or "fixable" problems (eg situations).

What do I need from the community here...?

To be heard? To know if anyone else out there has a long enduring preference to die even when they aren't feeling active distress? And if so, has anything helped them longterm to shift that thinking?

If anyone knows of any practitioners who *specialise* in chronic SI since a young age, I'd really love to know who (if there is a way someone is allowed to??). I've tried Google searching and using online psych directories and reading bio's. I've seen many psychologists and psychiatrists in the past who haven't been able to help me shift this (even if theyve helped in other ways).

I do wonder if my neural networks for positive emotional may not have fully developed or may have atrophied due to the young age and length of time. I genuinely do not know what being happy feels like, I do not have any memory that I recall as being "happy" or joy, and I suspect this makes recovery much harder.

Im ok at the moment. Im safe. But it is exhausting.

Apologies this post is quite a bit too long.

Re: TW: The 'real' me inside - I come with an SI warning

Sorry @Flutterbug9 , have tagged you in it now so it will be easy for you to find again from notifications 😊. I forget who is interested or not interested in the technical aspects of the trial info, and don't want to over-tag people because I know some people get overwhelmed and upset about it when it happens to them. Same as I didn't tag CS because I didn't want to overwhelm them with tags about the technical side of the trial, rather than how I go with it personally which I do know they are interested to keep in touch about. Sorry if you felt left-out or forgotten by not tagging in it; was not my intent. It was mostly in response to specific questions from CB, and I knew T has previously chatted with me about some stuff re: research trials.

Sending you extra care; I know you are hurting. I wish I could be there to hold you - and do lego with you too to co-regulate if that would help.

Oh, ouch - side of face sore. Possibly the jaw @Shaz51 ?

i hope the hot drink helps - possibly relax the muscles if that is at all causing it?

Or if it doesn't help, whether an ice pack might help if it is at all inflamed?

I'm just responding to your post about the trial @AlwaysMyself it sounds as though it's been pretty well thought out and there is a lot of clinician contact. It will be interesting to see how it goes. The treatment is very expensive though, so the trial will at least save you that money.

The place where I had the telehealth psychiatry appointment regarding TMS rang today wanting to make an appointment. I asked about the cost and they gave me a range for the out of pocket cost, when I asked why it's range they did not know. I don't think I can afford it, if I have to pay the high end of the out of pocket cost. Also the psychiatrist said it's best to do it without breaks and I have to find out when we are going up to my father's place for the next part of clearing up. So I have not made any appointments. I kind of felt like it's a bit of a money making exercise for these people, when I spoke to them today, so I felt more ambivalent about it. I am going to see my psychologist tomorrow and I can discuss it, I don't have anyone else I can discuss it with.

I did however, ask about psychedelic enhanced therapy and they said it's expensive and it's not recommended for people with cPTSD.

Hi @Shaz51 sorry to hear about your sore face, I hope the coffee helps to some extent. It is hot here too, but probably not as hot or at least as humid as where you are.

Re: TW: The 'real' me inside - I come with an SI warning

@Till23 interesting they gave the price as a range, indeed. I was given it as a "per session" rate - and of course the cost would be dependant upon how many sessions i did. I know you live a bit remotely, so I am wondering whether or not there is a second clinic within range that would be possible to enquire with instead? 🤔

Is there anywhere you could "go on holidays" for 6-7 weeks and get treatment during? Or if you have private health insurance possible a hospital that does it as part of an inpatient program? (and thus they co-pay most of it under insurance?) -- i know this would have to be seen if possible to schedule around sporting events/comps and things like your dad place being sorted out, and may not be possible but may be another idea?

Interesting re: psycadelics & cPTSD. I know they use it for PTSD, so it is interesting if they don't use it for complex PTSD.. maybe the risk of bringing up too much trauma at once is higher?

Re: TW: The 'real' me inside - I come with an SI warning

Hope it eases over the evening @Shaz51 . If it doesn't improve, do you think there is benefit it seeing if you can see a GP or Urgent Care Clinic nurse about it tomorrow? e.g. pain relief?

I am going to take a good leaf out of @Flutterbug9 's book and sit down to write out a dot-point summary of things for the MH GP I see next week for the first time, and I know I can re-use this for the trial doctor too when I meet him in a month.

I shall be off-forum for a number of hours (the whole evening?) - but I think this is a good time to do it, with my counsellor appointment being tomorrow if it does bring things up. 😊

Chat to you all later @Cuddlebear @Flutterbug9 @Shaz51 @Till23 😘 Thank you for being yourselves.
